高血压病患者的心功能评价

摘　　要：应用无创左右心功能同步检测仪测定原发性高血压患者２３４例的心功能参数与４０例正常人比较。结果显示心肌缺血阈值下降；左室射血分数、左室舒张末期压力、肺毛细血管平均嵌楔压、心肌耗氧量、左室舒张末期血量、左室射血阻抗和动脉弹性模量等较对照组升高（Ｐ＜０．０１）。原发性高血压的心脏早期病变主要表现为左室舒张功能障碍（３５％），其受损程度与年龄、病程无相关性。年龄６０岁左右的原发性高血压患者心率维持在５５～６９次／ｍｉｎ范围内，似有利于心肌供血。Investigationofcardiacfunctionalcapacityin234casesofessentialhyperten-sion(EH)camparedwith40normalsubjectsascontrolswasperformedwithanon-invasivesynchronousventricularfunctiondetector.Theresultsshowedthatleftventricularejectionfraction,leftventricularend-diastolicpressure,meanpulmonarycapilarywedgepressure,myocardialoxygenconsumption,leftventricularend-diastolicvolume,leftventricularejec-tionresistanceandmodulusofarterialelasticitywerehigherinpatientswithEHthaninthecontrols.Themyocardialischemicthresholdwaslowerthaninthenormalsubjects(P<0.01).Asmentionedabove,theleftventriculardiastolicdisturbance(LVDD)couldbeamainearlysigninEHwithasymptomaticheartfailure.TheseverityofLVDDinEHhadnosig-nificantrelationwiththecourseofilnesandpatientsages.ItissuggestedthatinEHpa-tientsaround60years,theheartrateof55～69beats/minseemsbeneficialforpreventionofmyocardialischemia.Thismethodismorespecific,mostsensitiveandcanbeusefulforearlydiagnosis,preventionandtreatmentinEHwithasymptomaticheartfailure.
