静注胺碘酮救治宽QRS心动过速25例疗效观察

摘　　要：目的:观察静脉注射胺碘酮对宽QRS心动过速的疗效及其安全性。方法:对25例宽QRS心动过速急诊患者,采用静注胺碘酮首剂150mg(3.5mg/kg)稀释于30ml生理盐水中15min内注入;随后以1mg/min维持静滴,观察30min无效再给150mg静注。连续心电监护于用药前及用药后5、10、15、30、120min测心律、心率和血压及观察其不良反应。结果:用药前后对比,心率有显著性差异(P﹤0.01),1d内有效率达92%(23/25),血压变化不大(P﹥0.05),未见明显不良反应。结论:静脉注胺碘酮救治宽QRS心动过速具有快速、安全、效果好的优点。可作为急诊救治宽QRS心动过速的首选药物之一。Objective To observe the efficacy and safety of amiodarone by infiusinon to treat tachycardia of wide-QRS. Method For 25 patients with tachycardia of wide-QRS,first dose of 150 mg of amiodarone in 30 ml of 0.9% N.S （3.5mg/kg） was given intravenously,followed by a maintaining dose of which at a speed of 1 mg/min.Another 150 mg of it was given if no efficacy was found within 30 minutes.Heart rate,rhytm and blood pressure were observed with continuous electrocardiographic monitoring , mean- while ,toxicities. Results After doses of amiodarone, heart rate dropped sigrfificangly （P〈 0.01）,with an effective rate of 92%（23/25） within 24 h, but blood pressure not（P〉 0.05）,and no toxicity was found. Conclusion amiodarone by infusion was quick, safe to treat patients with tachycardia of wide-QRS,it could be one of first-line drugs .
