MUC5 AC和MUC5 B在慢性鼻窦炎病变黏膜中的表达与分布研究

摘　　要：目的观察粘蛋白5AC 和5B(mucin,MUC5AC、MUC5B)在慢性鼻窦炎鼻窦黏膜中的表达及分布,探讨其在慢性鼻窦炎病理机制中的作用。方法取Ⅰ型和Ⅱ型慢性鼻窦炎患者上颌窦窦口黏膜各10例,另取10例正常上颌窦口黏膜作为对照,应用 HE 染色和免疫荧光方法观察各组鼻窦黏膜的病理改变,以及 MUC5AC和 MUC5B 在各组鼻窦黏膜中的表达情况,并通过计算阳性细胞面积比比较二者在各组鼻窦黏膜中的表达是否存在差异。结果Ⅰ型和Ⅱ型慢性鼻窦炎鼻窦黏膜均见杯状细胞和黏膜下腺体增生和化生。MUC5AC 和MUC5B 在两组病变的鼻窦黏膜中均有表达,前者主要表达于杯状细胞和假复层纤毛柱状上皮,后者主要表达于黏膜下腺体,两组间阳性细胞面积比差异均无统计学意义(P>0.05)。而正常对照组鼻窦黏膜中 MUC5AC 和MUC5B 均无表达或弱表达,阳性细胞面积比与上述两组间差异均有显著统计学意义(P<0.001)。结论MUC5AC 和 MUC5B 在慢性鼻窦炎的病理机制中有重要作用,与上皮杯状细胞和黏膜下腺体的增生和化生密切相关。Purpose To investigate the expression and distribution of MUC5AC and MUC5B in the sinus mucosa of chronic sinusitis and the possible mechanism in the pathogenesis of chronic sinusitis.Methods Twenty cases of chronic sinusitis （ 10 cases of type Ⅰ and 10 cases of type Ⅱ , respectively） and 10 normal cases were recruited in this study. The pathologic changes and expression of MUC5AC and MUC5B in the sinus mucosa among these 3 groups were determined with the methods of HE and immunofluorescence. Ratios of positive cell areas were compared among these 3 groups. Results Hyperplasia and metaplasia of goblet cells and submueosal glands were observed in all cases of type Ⅰ and type Ⅱ chronic sinusitis. Meanwhile, MUC5AC and MUC5B were expressed in the other 2 groups, whereas weak or negative expression in the normal group. The strong expression of MUC5AC in group type Ⅰ and type Ⅱ was in goblet cells and pseudostratified ciliated columnar epithelia , while MUC5B was mainly expressed in submucosal glands in these 2 groups. There were no statistical differences between the ratios of positive cell areas in the other 2 groups（ P 〉 0.05）, and significant statistical differences was analyzed between the normal group and the other 2 groups （P 〈 0.001 ） .Cnclusion MUC5AC and MUC5B play an important role in the pathogenesis of chronic sinusitis and are associated with hyperplasia and metaplasia of the goblet cells and submucosal glands. （ Chin J Ophthalmol and Otorhinolaryngology, 2006,6： 217 - 219）
