摘  要:[目的]建立猪腰段脊髓火器伤模型和改良A llen's打击伤后全瘫模型,观察伤后肾脏病理和肾功能的早期变化。[方法]动物随机分为两组,火器伤组(G)6只,在全麻状态下做L1~2脊髓火器伤模型;打击伤组(A)6只,L1节段脊髓行改良A llen's打击。两组动物分别于伤前(0 h)、伤后0.5 h、1 h、2 h、4 h检测血肌酐和血尿素氮(BUN)。死亡动物行肾脏病理检查。[结果]A组BUN值在伤后4 h升高,G组BUN值在伤后1 h升高。两组肌酐在伤后0.5 h升高,各点监测G组明显高于A组。G组动物伤后5.2 h^7.5 h死亡,死亡率为100%。病理检查示G组肾包膜及肾实质广泛充血、出血。[结论]G组火器伤较A组肾脏损伤及组织破坏严重,在伤后早期出现肾功能变化。脊髓火器伤后肾功能变化早期应给予积极处理,以降低早期死亡率。[ Objective] To investigate the early changes of kidney pathology and the figure of renal function in porcine gunshot injury of lumbar spinal cord. [ Method] Twelve healthy porcine were randomly divided into two groups: gunshot injury group (G gToup,n =6) and the Allen method induced injury group (A group, n =6). We monitored the figure of blood urea and creatinine at pre-injury point and 0.5,1,2,4 hours post-injury, and the renal pathology of the dead animals was examined. [ Result] The mortality of Gunshot gToup (G) was 100% , the survival time interval ranged from 5.2 h to 7.5 h. The figures of blood urea of Group G increased from the post-injury period of 1 hr, while the corresponding figures of Group A increased from 4 hr. The figures of creatinine of both groups grew from 0.5 hr, which that of Group A stabilized relatively. [ Conclusion] Gunshot injury with specific mechanism has done more damage to body than Group A, including more extensive local injury and kidney damage, and intensive care should be payed for early renal damage after gunshot of spinal cord to lowering the early mortality.
