彩色多普勒超声心动图对中间型房室间隔缺损的诊断

摘　　要：目的探讨彩色多普勒超声心动图对中间型房室间隔缺损的诊断价值。方法应用彩色多普勒超声心动图检查15例中间型房室间隔缺损患者,观察其超声心动图特征,与手术所见进行对照。结果15例患者均经手术治疗并证实术前超声心动图诊断。中间型房室间隔缺损的彩色多普勒超声心动图特征包括:①所有15例患者均存在原发孔型房间隔缺损及流入道室间隔缺损;②所有患者房室瓣分为单独的二尖瓣和三尖瓣,二尖瓣前叶与三尖瓣隔叶位于同一平面,接近半数患者存在二尖瓣或/和三尖瓣裂;③CDFI及频谱多普勒均清晰显示房室水平分流及房室瓣反流。结论彩色多普勒超声心动图对中间型房室间隔缺损具有重要诊断价值。Objective To evaluate the diagnostic value of intermediate type of atrioventricular septal defect using color Doppler echocardiography. Methods Fifteen patients with intermediate type of atrioventricular septal defect were studied by color Doppler echocantiography and the echocardiographic characteristics were analysed. Results All 15 cases were accurately diagnosed by color Doppler echocantiography. The most important echocardiographic characteristics of intermediate type of atrioventricular septal defect included：①A large atrioventricular defect consisted of a premium atrial septal defect and an inlet ventricular septai defect. ②Two separate atrioventricnlar valves were at the same level. Nearly half of the patients had cleft mitnd or/and tricuspid valves. ③CDFI demonstrated all patients had significant atrioventricular valvular regurgitation. Conclusion Color Doppler echocardiography has important value in diagnosis of intermediate type of atrioventricular septal defect.
