摘  要:目的探讨早期进食对肝移植病人术后恢复的影响。方法用随机数排序分组法将2005年1-9月实施原位肝移植术的52例病人随机分为早期进食组和常规进食组,各26例,比较两组病人术后感染率、死亡率、肝功能、胃肠道功能恢复情况、住ICU时间、营养费用、睡眠质量。结果早期进食组肝移植术后感染率、营养费用明显低于常规进食组;肝功能、睡眠情况恢复较好;胃肠道功能恢复时间、住ICU时间较常规进食组短,两组比较均有显著性差异,P<0.05或P<0.01。营养指标和死亡率两组无显著性差异,P>0.05。结论早期进食是安全可行的,与常规进食相比,早期进食减少感染率,促进肝功能和胃肠功能恢复,改善睡眠质量,缩短ICU监护时间,降低营养费用。Objective To investigate the effects of early-stage feeding on recovery of patients after liver transplantation. Methods 52 patients after orthotopic liver transplantation from January to September 2005 were divided into early-stage feeding group and conventional feeding group randomly, with each group having 26 patients. The incidence of infection, mortality, the liver functions, recovery time of gastrointestinal function, the time length of ICU, nutritional expenses and sleeping quality of patients in two groups were compared. Results The incidence of infection and nutritional expenses in the patients of early-stage feeding group were significantly lower than those in control group; the liver functions and sleeping quality were better then those in control group; recovery time of gastrointestinal function and the time length of ICU were significantly shorter. All these showed significant differences between two groups(P〈0.05 or P〈0.01). There were no significant difference in nutritional parameter and mortality between the two groups(P〉0.05). Conclusion Early-stage feeding conforms to physiological need and it is safe and feasible.
