代谢综合征患者大动脉顺应性与颈动脉内-中膜厚度变化的研究

摘　　要：目的探讨代谢综合征(MS)患者大动脉顺应性及颈动脉内-中膜厚度(IMT)的变化。方法64例MS患者和56例年龄相匹配的正常对照者,测定所有入选者颈动脉-股动脉的脉搏波速度(PWV)、颈动脉IMT,同时检测其身高、体重、腰围、臀围、血压、血糖、血脂水平,并计算体重指数(BMI)、腰臀比(WHR)。结果MS组PWV和颈动脉IMT与对照组比较,差异有统计学意义(P<0.05)。MS组颈动脉IMT与年龄、腰围、收缩压、脉压、总胆固醇、甘油三酯、PWV呈显著正相关(P<0.01),与高密度脂蛋白-胆固醇呈显著负相关(P<0.01);PWV与年龄、收缩压、脉压、体重指数、总胆固醇、甘油三酯、颈动脉IMT、体重呈显著正相关(P<0.05),与高密度脂蛋白-胆固醇呈显著负相关(P<0.01)。结论MS患者PWV及颈动脉IMT均明显升高,已经存在大动脉顺应性的减退;MS患者大动脉顺应性的变化与年龄、血压、肥胖及血脂紊乱有关,且PWV与颈动脉IMT有明显的相关性。Objective To investigate the changes of comphance in large arteries and carotid artery intima-media thickness （IMT）in patients with metabolic syndrome. Methods There were 64 patients with metabolic syndrome and 56 age -matched control subjects. Their carotid-femoral pulse wave velocities （C-FPWV）were measured by the Comphor SP and their carotid artery IMT were detected by B-mede ultrasound.At the same time their height, weight, waist circumstance, hip girth, blood pressure, blood glucose, blood hpid, BMI and waist to hip ratio （WHR） were measured. Results Compared with the control, the patients with metabohc syndrome had higher C-FPWV （P〈0.01）and higher carotid artery IMT （P 〈 0.05）. Correlation analysis demonstrated that the carotid artery IMT was positively correlated with age, WHR, systolic blood pressure（SBP）, pulse pressure （PP）, total cholesterol, triglyceride,PWV（P 〈 0.01）, but negatively correlated with HDL-C（P 〈 0.01）. Correlation analysis also demonstrated that C-FPWV was positively correlated with age, systolic blood pressure （SBP）, pulse pressure （PP）,BMI, total cholesterol, triglyceride, IMT, weight（P 〈 0.05）, but negatively correlated with HDL-C（P 〈 0.01） Conclusions There is the decrease of large arterial compliance in patients with metabohc syndrome. The changes of large arterial comphance are correlated with the age, blood pressure, obesity and the disorder of blood lipid. The C-FPWV in patients with metabolic syndrome was correlated with carotid artery IMT.
