聚焦超声治疗外阴上皮内非瘤样病变疗效评价

摘　　要：目的：探讨聚焦超声治疗外阴上皮内非瘤样病变的临床疗效。方法：经病理证实外阴上皮内非瘤样病变75例，其中增生型36例，硬化苔癣型28例，混合型11例。年龄12～69岁，平均36．81岁；病程〈3年10例，3～10年48例，〉10年17例。依患者意愿分为超声组45例，对照组30例。超声组采用聚焦超声，治疗功率为4．0W，连续直线扫描方式，治疗时间20-40min，通常治疗1次。对照组用药物治疗，使用3个月。于治疗后第1、3和6个月依近期治愈标准判断疗效。结果：超声组临床治愈12例（26．7%），显效28例（62．2%），好转5例（11．1%），总有效率为100％，无复发。对照组：临床治愈1例（3．3%），显效5例（16．7%），好转19例（63．3%），无效4例（13．3%），但停止治疗1～2个月，复发21例（70．0%）。结论：聚焦超声治疗外阴上皮内非瘤样病变是快速、安全有效的。Objective To investigate the clinical curative effects of focused ultrasound treatment for nonneoplastic epithelial disorders of vulva. Methods Seventy-five patients （aging 12-69, mean 36. 81 years） with nonneoplastic epithelial disorders of vulva were confirmed by pathology, including thirty-six patients with squamous hyperplasia, twenty-eight patients with lichen sclerosus accompanied and eleven patients with lichen sclerosus accompanied with squamous hyperlasia. All of them were randomly assigned to two groups. 45 cases in the ultrasound group were treated with Seapostar （model： CZF） ultrasound treatment instrument. The power of 4.0 W was used for 20- 40 min in each session. The other 30 patients were received medical routine treatment. A duration of 90 d was treatment course. Results All patients in ultrasound group recovered at different degrees , among them 12 cases （26.7%） were cured, 28 cases （62. 2%） were obviously improved, 5 cases （11. 1%） were improved. The effective rate was 100 %. The pathological changes indicated that the squamous epithelium of vulva has recovered normal stratification and thickness. In the control group, 1 case （3.3%） was cured, 5 cases （63.3%） were obviously improved and 19 cases （16.7%） were improved. But 21 cases （70. 0%） were found relapsed pruritus of vulva 1- 2 months after treatment. Conclusion The focused ultrasound treatment is a new effective and safe method for nonneoplastic epithelial disorders of vulva.
