摘 要:【目的】探讨B型脑钠肽(BNP)在鉴别老年患者支气管哮喘急性发作与心源性哮喘中的作用。【方法】测定71例确诊为支气管哮喘的老年患者急性加重期和缓解期血浆BNP水平并与65例心源性哮喘患者进行比较,同时对比分析其动脉血氧分压(PaO2)、平均动脉压(MAP)、心率(HR)的关系。【结果】支气管哮喘患者急性加重期血浆BNP水平(109.98±13.74)ng/L显著低于心源性哮喘(1 125.63±302.54)ng/L(P<0.01)。在心源性哮喘控制后72 h的BNP浓度与哮喘急性发作期比较,差异亦有显著性(P<0.05)。【结论】BNP能较好地鉴别老年患者支气管哮喘急性发作与心源性哮喘,其过程简单方便,值得临床推广应用。[Objective]To construct a new possible method for distinguishing asthma and acute cardiac insufficiency in the elderly. [Methods]Peripheral blood brain natriuretic peptide(BNP),PaO2 were measured in 71 patients with elderly asthma and 65 patients with acute cardiac insufficiency. Their MAP,HR recorded in acute exacerbation and relaxing period. [Results]The level of plasma NNP decreased obviously in asthma on acute exacerbation compared with that in acute cardiac insufficiency ( P 〈0.01 ), and difference in relaxing period of the two groups was significant( P 〈0.01). When the level of plasma BNP was dropping, clinic symptoms improved in patients with acute cardiac insufficiency . [Conclusion]It is an effective method to measure peripheral blood BNP to distinguish asthma and acute cardiac insufficiency in the elderly .
