老年良性前列腺增生患者常规联合尿动力学检查的临床价值

摘　　要：目的探讨老年良性前列腺增生(BPH)患者常规联合尿动力学检查的临床价值。方法选取有下尿路症状的BPH患者251例,按年龄分为A组(≥65岁,176例)和B组(<65岁,75例)。再分别把两组中不伴有糖尿病、脑血管疾病和帕金森氏病的患者分为A1组(85例)和B1组(64例)。A1组和B1组按世界尿控协会推荐的标准方法行尿动力学检查。结果A组伴糖尿病、脑血管疾病和帕金森氏病者91例(51.7%),B组11例(14.67%),差异有统计学意义(P<0.001)。A1组有因急性尿潴留而留置尿管或膀胱造瘘病史者29例(34.12%),B1组8例(12.50%),差异有统计学意义(P<0.01)。A1组最大尿流率显著低于B1组,逼尿肌收缩乏力或没有主动收缩发生率显著高于B1组,差异有统计学意义。结论有下尿路症状(LUTS)的老年BPH患者容易发生逼尿肌收缩功能障碍,常规联合尿动力学检查,有助于提高治疗效果。[Objective] To investigate the value of routine urodynamics in elder men with benign prostatic hyperplasia （BPH）. [Methods] 251 patients with BPH and lower urethral tract symptoms were divided into two groups according to ages. group A （176 cases,≥65 years） and group B（75 cases, 〈65 years）. Ptients without diabetes, cerebrovascular disease, Parkinson＇s disease from group A and group B were divided into two subgroups ： group A1 （85 cases） and group B1 （64 cases） respectively. Group A1 and group B1 were performed urodynamic studies. [Results] The patients in group A （91/176,51.7 %） were found to have higher prevalence of diabetes, cerebrovascular disease, Parkinson＇ s disease than those in group B （11 of 75,14.67 %） （ P 〈 0. 001） More patients in group A1 had history of catheterization or cystostomy because of acute urinary retention than those in group B1 （34.12% ： 12.50%, P 〈0. 01）. The maximum free flow rate of group A1 was lower than that of group B1, and the significant difference incidence of detrusor underactivity or detrusor acontractile was found between group A1 and group B1. [Conclusion] The elder men with BPH and LUTS tend to have detrusordysfunction. However, treatment effect can be improved by performing routine urodynamic.
