PICP和PC Ⅲ评价高血压心肌纤维化的临床价值

摘　　要：目的探讨血清I型前胶原羧基端肽(P ICP)、Ⅲ型前胶原(PCⅢ)水平作为高血压病(EH)心肌纤维化(M F)血清标志物的临床价值。方法采用放免法测定41例EH左室肥厚(EH-LVH组)和53例EH非左室肥厚(EH-NLVH组)及32例正常人(对照组)血清P ICP、PCⅢ水平;用二维超声及多普勒技术测量左室重量指数及舒张功能参数[二尖瓣E波与A波峰值速度(VE、VA)、左室等容舒张时间(IVRT)]。结果①EH-LVH组和EH-NLVH组P ICP、PCⅢ浓度均显著高于对照组,EH-LVH组均显著高于EH-NLVH组;EH-LVH组及EH-NLVH组VE/VA较对照组显著降低;EH-LVH组IVRT较对照组及EH-NLVH组显著延长,EH-NLVH组较对照组显著延长。②P ICP与VA正相关,r=0.301,P<0.01;PCⅢ与VE负相关,r=-0.329,P<0.01;P ICP、PCⅢ均与IVRT呈正相关,分别为r=0.324,P<0.01;r=0.513,P<0.001。结论血清P ICP、PCⅢ可作为评价M F及反映左室舒张功能的敏感参数。[Objective] To investigate the clinical value of quantitative assessment of MF in essential hypertension （EH） patients by serum concentrations of carboxyterminal propeptide of procollagen type Ⅰ （PICP） and procollagen type Ⅲ （PC Ⅲ ）. [Methods] The study was performed in EH patients, 41 cases with left Ventricular Hypertrophy （EH-LVH）, 53 cases without left Ventricular Hypertrophy （EH-NLVH）and 32 normotensive control subjects. The serum concentrations of PICP and PC Ⅲ were measured by specific radioimmunoassay. Two-dimensional, M-mode and Doppler ultrasound recordings were obtained in every subject to determine several parameters of the left ventricle anatomy and function. [Results ] （1）The serum concentrations of PICP and PC Ⅲ had significant differences among three groups, P 〈 0. 05. （2）VE/VA was significantly decreased （P〈0. 05） in groups EH-LVH and EH-NLVH compared with normotensive subjects. IVRT had significant difference among three groups, P〈0. 05. （3） The serum concentration of PICP was correlated positively with VA （r=0. 301,P〈0. 01）. The serum concentration of PC Ⅲ was correlated negatively with VE （r=-0. 329,P〈0. 01）. The serum concentration of PICP and PC Ⅲ were correlated positively with IVRT. [Conclusion] The serum concentrations of PICP and PC Ⅲ are useful marker in determination of the degree of MF with EH. They can be used to evaluate left ventricular diastolic function.
