摘  要:目的:观察宫腔镜电切术的病人术中灌流液吸收量与术后血生化(钾、钠、氯、血糖、渗透压等)的变化,分析探讨宫腔镜电切术的安全性。方法:选择2004年9月-2005年5月在我院进行宫腔镜电切术的病人共68例,于术中记录灌流液的吸收量,术前、术后1h抽取静脉血进行生化检测,对比手术时间、灌流液吸收量对血生化变化的影响。结果:68例宫腔镜电切术中无1例出现经尿道电切前列腺(TURP)综合征。血钠随灌流液的吸收量增加逐渐下降(P〈0.01),血钾、氯、血浆渗透压无明显变化(P〉0.05),11.7%的病人血糖于术后升高;血钾、氯、血糖及血浆渗透压变化与灌流液的吸收无相关性(P〉0.05);血钠变化与灌流液的吸收有相关性(P〈0.05)。结论:宫腔镜电切术是安全的,只要做好术中监护,控制手术时间,就可避免或减少水中毒并发症的发生。Objective: To evaluate the safety of the hysteroscopy surgery by analyzing the absorption of irrigation fluid and post -operation hematological biochemical (K^+ , Na^+ , Cl^-, Glu, plasma osmotic pressure, etc. ) index changes. Methods: 68 patients were adopted hysteroscopy surgery between SOp 2004 and May 2005. A change of hematological biochemical index was detected before and 1 hour after the operation. Then the factors were compared as the operation method, length and absorption of irrigation fluid. Results: No TURP Syndrome occurred among the 68 patients after the operation. Among all the parameters, only the changes of Na^+ were related on the absorptions of irrigation fluid (P 〈0. 01 ) . And there are no obvious change in K^+ , Cl^- and plasma osmotic pressure (P 〉0. 05) . 11.7% patients' blood sugar increased post- operation. Concluusion: Hysteroscopie surgery is safe. But the wardship should be enhanced during the operation and the length of time should be eontroled. So the occurrence of the complications related on overage absorption of irrigation fluid can be avoid or reduced.
