丙种球蛋白治疗小儿支原体肺炎合并心肌损害的疗效观察

摘　　要：目的观察丙种球蛋白（IVIG）治疗小儿支原体肺炎合并心肌损害的效果。方法将支原体肺炎合并心肌损害患儿115例随机分为2组，均给予常规治疗，并于入院后1～3d给予地塞米松静滴。治疗组在此基础上给予IVIG0．5g／（kg·d）静滴，连用3d，在给定的时间内检测相应指标。结果至入院后第15天统计，治疗组治愈例数（45例）明显高于对照组（17例）（P〈0．05）。治疗10d时，除临床症状外，治疗组其他各项参考指标改善情况均优于对照组（P〈0．05）。治疗15d时，治疗组及心电图转为正常例数均明显高于对照组（P〈0．01和0．05），且绝大多数患儿的心肌酶降为正常，与同时间点对照组比较，有显著性差异（P〈0．01）。结论IVIG治疗小儿支原体肺炎合并心肌损害能显著减轻组织破坏，疗效确切。Objective It is to observe the therapeutic effects of intravenous γ- globulin （IVIG） on mycoplasma pneumoniae （MP） pneumonia complicating with myocardial damage. Methods 115 children with MP pneumonia complicating with myocardial damage were randomly divided into control group and treatment group. Both the groups were treated with normal therapy, and given dexamethasone after being in hospital for 1 - 3 days. On the basis of treatment above, treatment group were additionally given IVIG 0.5 g/（kg·d） for 3 days. All children were observed and accepted the corresponding laboratory examination at the given time. Results The cures （45 cases） in treatment group were more than those （17 cases） in control group after 15 days＇ hospitalization （ P 〈 0.05）. The improvements of all the referred indexes except clinical symtoms in treatment group were better than those in control group after 10 days＇ treatment （P〈 0.05）. Compared with control group at the same time, the cases of normal ECG were still higher after 15 days＇ treatment （P 〈 0.05 and P〈 0.01）, and the levels of cardiac muscle enzymes of most of child patients lowered to normal in treatment group. Conclusion γ-globulin could significantly alleviate histological injury in children with MP pneumonia complicating with myocardial damage.
