全结肠、直肠重复畸形四例诊治经验

摘　　要：目的提高对全结肠、直肠重复畸形的认识,减少误诊。方法对4例全结肠、直肠重复畸形的诊治进行总结。结果3例重复肠管开口在舟状窝,1例重复肠管远端为盲端。4例中2例合并泌尿生殖畸形,其中1例同时合并先天性肛门闭锁,其重复肠管与主肠管分别开口在舟状窝处,术前误诊。3例行主肠管与重复肠管的开窗术,1例行全结肠切除术。4例术后均恢复良好。结论全结肠、直肠重复畸形常合并有泌尿、生殖系统畸形,钡灌肠X线检查可确诊。治疗可采用全结肠切除、重复肠管黏膜剥脱及重复肠管与主肠管开窗术。Objective To decrease the misdiagnosis of patients with full length duplications of the colorectum. Methods We retrospectively analyzed the clinical data of 4 cases of full length duplication of the colorectum. Results The terminal end of the duplicated intestine opened into rectovestublar in three cases and the terminal end was blind in one patient. Among the four cases, two suffered from combined ufinary-genit malformation, one of which was with imperforate anus and its functional bowel and duplicated bowel were all opened on the rectovestublar, this case was misdiagnosised preoperatively. Windowing was performed between dupilcated and functional bowel in three cases. Conclusion Duplications of whole colon-rectum are rare diseases. Urinary-genital malformation often complicated with it. Diagnosis could be established by barium enema. Total colectomy, windowing and mucodenudation are the choice of therapy according to different clinicopathologic characters.
