踝前皮瓣的临床解剖与手术设计

摘　　要：目的:为踝前皮瓣移位与移植提供解剖学基础。方法:20例红色乳胶灌注的成人下肢标本,在手术显微镜下解剖观察踝前皮瓣供血血管。结果:足背动脉在踝前伸肌下支持带上缘、中点("Y"形韧带分杈处)及下缘各有一分支发向踝前皮肤。皮支起始处血管直径分别为(0.12±0.05)mm、(0.20±0.07)mm和(0.13±0.04)mm。在伸肌下支持带中点分杈处发出的皮支15%起源于跗外侧动脉。结论:踝前区可以设计以踝前皮支为蒂的带蒂皮瓣,也可以设计以踝前足背动脉为蒂的踝前皮瓣游离移植。Objective：To provide the anatomic foundation for shifting and transplanting of anterior malleolus flap. Methods： The supply vessels of anterior malleolus flap were observed under operating microscope on 20 sides of adult low extremity specimens perfused with red latex. Results： The branch of the arteria dorsalis pedis extended to anterior malleolus skin from the superior border, middle point （the crotch on Y-like ligament） and inferior border of the inferior extensor retinaculum. The diameter of cutaneous branch at the original site was about （0.12±0.05）mm,（0.2±0.07）mm and （0.13±0.04）mm respectively. Sometimes the cutaneous branch from the middle point of the crotch originated from lateral tarsal artery（ 15% ）. Conclusions： The anterior talocrural region can be designed as pedicled skin flap with cutaneous branch, or flee skin flap with arteria dorsalis pediwithanine.
