老年2型糖尿病胰岛素抵抗和胰岛β细胞功能的初步探讨

摘  要:目的探讨老年2型糖尿病患者胰岛素抵抗和胰岛β细胞功能的特点。方法测定30例老年2型糖尿病患者空腹血糖(FPG)、血清胰岛素(FNS)和C肽,计算HOMA-IR和胰岛素敏感指数(ISI)以评价胰岛素抵抗,以HOMA-β和C肽作为衡量胰岛β细胞功能指标,与10例健康老年人的相应指标作对比并进行分析。结果与对照组相比,老年2型糖尿病组的FPG、FNS和HO-MA-IR明显升高,而C肽I、SI和HOMA-β明显下降,其间差异有显著意义(P<0.05)。结论老年2型糖尿病患者存在明显的胰岛素抵抗和胰岛β细胞功能异常,同时可能伴有高胰岛素低C肽分离现象,提示胰岛素抵抗和胰岛β细胞功能异常是糖尿病患者糖代谢紊乱的主要原因。Objective To study the characteristics of insulin resistance and the function of islet cells in elderly patients with type 2 diabetes mellitus(T2DM). Methods Fasting plasma glucose(FPG), fasting serum insulin(FNS) and C-peptide were measured in 30 elderly patients with type 2 diabetes mellitus respectively. Homeostasis model assessment-insulin resistance index(HOMA-IR) and isulin sensitivity index (ISI) were applied to assess the status of insulin resistancy, HOMA-β was used to assess the basic function of isletfl cells. These values were compared with 10 healthy seniors. Results The level of Cpeptide and ISI and HOMA-flwere lower than in control group. There were very significant differences between T2DM group and the control group (P〈0. 05). Conclusion Elderly patients with type 2 diabetes mellitus have obvious insulin resistance and islet cells dysfunction. Hyperisulinemia with low C-peptide level may exist in elderly patients with type 2 diabetes mellitus. This study suggests that insulin resistance and islet cells dysfunction may play a key role in glycometabolism disorder in elderly patient with type 2 diabetes mellitus.
