孟鲁司特钠治疗和预防73例儿童咳嗽变异性哮喘的疗效观察

摘　　要：目的:探讨孟鲁司特钠治疗及预防儿童咳嗽变异性哮喘的疗效。方法:对73例咳嗽变异性哮喘(cough variant asthma,CVA)患儿,按2:1比例随机分成治疗组49例和对照组24例。对照组接受盐酸班布特罗和氯雷他定片治疗,至症状消失后停药,结束后随访12个月观察咳嗽症状恶化数和转变成典型哮喘的比例。治疗组在对照组治疗基础上加用孟鲁司特钠,至症状消失后停用盐酸班布特罗和氯雷他定,继续服用孟鲁司特钠,总疗程12周,停药后再随访12个月观察咳嗽症状恶化数和转化成典型哮喘的比例。对于6岁以上的32例患儿,于治疗前和治疗后3个月,均给予FEV1和PEF测定,比较两组患儿治疗前后肺功能变化情况。结果:治疗组取得临床缓解所需时间(T)比对照组明显缩短,且咳嗽症状恶化数和典型哮喘转化率均较对照组明显降低;用药3个月,治疗组肺功能较治疗前明显改善(P<0.01)。结论:孟鲁司特钠治疗及预防咳嗽变异性哮喘疗效肯定,副作用少,患者依从性好。Objective： To study curative effect of Montelukast in patient with CVA （ cough variant asthma）. Method： 73 patients with CVA were assigned to two groups in the ratio of 2：1. The study group has 49 cases and the others belonging to the control group. The control group were treated with Bambuterol Hydrochloride and Loratadine. The treatment withdraw until the symptom disappeared, the recurrence rate of CVA and incidence of CVA - Asthma conversion were investigated at the follow - up of 12 months. The study group was treated with Montelukast besides Bambuterol Hydrochloride and Loratadine. Bambuterol Hydrochloride and Loratadine were with drawn until the symptom disappeared, however, Montelukast was still taken till the course of treatment reached 12 weeks. The incidence of CVA recurrence rate and CVA - Asthma conversion were also investigated at the follow -up of 12 months. For 32 patient who were older than 6 years , the FEVI and PEF were measured before treatment and 12 weeks after treatmen. Result： The remission time of study group was shorter than the control group; the recurrence rate and incidence of CVA - Asthma convension are lower. Lung function in study group improved significantly compared to the control group after 12 weeks - treatment （ P 〈0.01. Conclusion ：The curative effect of Montelukast in patients with CVA was significant, with little side effect and good obedience.
