摘  要:目的探讨血氨(NH3)、胆碱酯酶(CHE)在人工肝支持系统(ALSS)治疗前后的变化及其临床应用。方法用干化学法快速检测NH3、CHE,同时用速率法检测丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST),对正常对照组110例和该院传染科56例重型肝炎患者ALSS治疗前后血标本,分别进行四项指标检测,并对结果进行统计分析。结果重型肝炎患者进行人工肝(ALSS)治疗后血NH3明显下降(P<0.05);血CHE明显升高,差异有统计学意义(P<0.01)。血清ALT、AST治疗后与血氨变化呈一致性,治疗前后差异有统计学意义(P<0.05)。各项指标与正常对照比较,差异均有统计学意义(P<0.05或P<0.01)。结论ALSS治疗后肝功能有显著改善,血NH3、CHE采用干化学法能快速检测,适合急诊使用,可随时监测。因此通过观察血NH3、CHE水平变化,能对ALSS治疗的疗效和预后状况进行及时监测和采取相应的治疗措施。Objective To discuss clinical application and variety of NH3 and CHE before and after treating by artificial liver supported system (ALSS) therapy. Methods NH3 and CHE were tested by fast dry chemical method, then ALT and AST by kinetic method, the four index tests of 110 normal people's blood samples as compared group and 56 blood samples from severe hepatitis patients who were in the hospital before and after treating, at last all the results were analyzed. Results NH3 level of severe hepatitis patients declined distinctly after treating by ALSS (P〈0.05), but CHE level raised obviously, h had distinct difference (P〈0.01) ,the variety of ALT and AST was the same as NH3 after treating, the difference was meaningful for analysing(P〈0.05). Comparing all the results between the severe hepatitis patients and normal group, there was distinct difference(P〈0.05 or P〈0.01). Conclusion The liver function improves a lot after treating by ALSS. Testing of NH3 and CHE are tested by dry chemistry fast test, it is suitable for emergence and convenience for surveillance. Through observing the level variety of NH3 and CHE in the blood, it can supervise and take some therapy method for the effect of ALSS treatment and the prognositic situation.
