儿童过敏性紫癜肾炎血清VEGF和TNF-α水平及临床意义

摘　　要：目的观察过敏性紫癜肾炎(HSPN)患儿血清中血管内皮生长因子(VEGF)和肿瘤坏死因子-α(TNF-α)水平的变化,并探讨其与临床的关系。方法检测36例HSPN患儿治疗前后血清VEGF和TNF-α水平,另设36例健康儿童为对照组,比较治疗前后血清VEGF和TNF-α水平的变化及其与临床的关系。结果HSPN患儿血清VEGF和TNF-α水平升高,显著高于对照组P<0.01。经合理治疗后,VEGF和TNF-α水平均显著下降。VEGF和TNF-α水平与病情相关,轻、中型患儿血清VEGF水平升高,显著高于对照组,但重型患儿VEGF水平反而下降,显著低于对照组;重型患儿血清VEGF水平也显著低于轻型和中型患儿,轻型与中型比较,轻型也明显高于中型者。重、中型患儿血清TNF-α水平都显著高于正常对照组,轻型患儿TNF-α水平虽有升高,但与正常对照组比较无统计学差异;重型患儿TNF-α水平也显著高于中型和轻型者,中型也显著高于轻型者。结论VEGF和TNF-α在HSPN的发生、发展中发挥一定的作用,血清VEGF和TNF-α水平在一定程度上反映了病情的轻重和预后。Objective To observe the changes of vascular endothelial growth factor （VEGF） and tumor necrosis factor α （TNF-α） in serum of children with Henoch-Schonlein purpura nephritis （HSPN） and explore the relationship between VEGF , TNF-α and the clinical characteristics. Methods The concentrations of VEGF and TNF-α in serum were detected with enzyme-linked immunosorbent assay in 36 children with HSPN and 36 healthy children. Results The concentrations of VEGF and TNF-α in HSPN children were significantly higher than that in healthy children （P（0.01）, After rational therapy, the concentrations of VEGF and TNF-α were significantly decreased. The concentrations of VEGF and TNF-α were correlated with the patients＇ condition, The concentrations of VEGF in mild and moderate patients were significantly increased, but decreased in severe patients. In severe patients, it was significantly lower than that in mild and moderate patients. In mild patients, it was also higher than that in moderate patients. The concentrations of TNF-α in all patients were increased. In severe or moderate patients, it was significantly higher than that in control group. In mild patients, it was also increased, but it had no difference between them and control group. In severe patients, it was higher than that in mild and moderate patients, and higher in moderate patients than that in mild patients. Conclusions VEGF and TNF-α played a role in the development and progress of HSPN. The concentrations of VEGF and TNF-α in serum reflected the patients＇ conditions and prognosis.
