摘 要:目的探讨血清半胱氨酸蛋白酶抑制剂C(Cystatin C,Cys C)的浓度在判定肾小球滤过功能中的价值。方法乳胶颗粒增强比浊法检测143例肾病患者血清Cys C浓度,用全自动生化分析仪检测血尿素氮(BUN)、血清肌酐(Scr)和尿肌酐,并计算24 h尿肌酐清除率(CCr)。根据血肌酐值(Scr)分成不同肾功能组。结果随着肾功能的减退,血清Cys C、BUN和Scr值逐渐升高,CCr值逐渐下降;Cys C与BUN、Scr、CCr均有相关性,其中与CCr的相关性最好;在肾功能正常组(Scr<132μmol/L)的患者中,Cys C可以反映早期肾小球滤过功能的下降。结论Cys C是一种比Scr更好的反映肾小球滤过功能的指标,能反映早期肾功能的损害而且检测方法简便、敏感及准确。Objective the concentration of ( Cys C ) evaluate serum cystatin C for estimation of glumerular filtration function. Methods Cys C concentration was measured by using latex particle enhanced turbidimetry in 143 cases of nephrology. BUN, serum creatinine ( Ser ) and urinary creatinine were detected by automatic biochemistry analyzer. 24-hour urine creatinine clearance ( Ccr ) was calculated. According to serum creatlnine levels, the patients were divided into different renal function impairment groups. Results With the decrease of the renal function, the values of serum cystatin C, BUN and Scr were found to increase with decreasing values of Ccr. BUN, Scr and Ccr were all correlated with cystatin C concentration significantly. The correlation of Cys C and Ccr was superior to that of others and CCr. In patients with normal renal function( Ser 〈 132 Nmd) ,Cys C concentration could show early renal impairment. Conclusion Serum cystatin C is superior to serum creatinine as a marker of renal function and can show early renal impairment.
