喹硫平治疗老年期痴呆患者行为和心理症状的临床观察

摘　　要：目的:探讨喹硫平治疗痴呆的行为和心理症状的疗效及不良反应。方法:64例痴呆患者随机分成两组,分别使用喹硫平和利培酮治疗8周;用AD病理行为评分表评价疗效,药物副作用量表评价不良反应。结果:(1)喹硫平组有效率为87.5%(28/32),利培酮组有效率84.4%(27/32),两组总有效率相当(P>0.05);(2)喹硫平组治疗8周后各因子分均显著降低,其中攻击行为、日夜节律紊乱、焦虑和恐惧、行为紊乱4个因子的减分具有极显著差异(P<0.01);与对照组相比在攻击行为、行为紊乱2个因子的减分的差异具有显著性(P<0.05);(3)两组不良反应率相当。结论:喹硫平和利培酮治疗痴呆的行为和心理症状的疗效相当,喹硫平的优势在于对行为障碍的疗效更加明显。Objective：To explore the curative effects and side effects of quetiapine in the behavioral and psychological symptoms of dementia. Methods：64 patients with dementias were randomly assigned to two groups that were treated with either quetiapine or risperidone for 8 weeks. The therapeutic response and side effects were evaluated with the behavioral pathology in Alzheimer＇s disease rating scale （ BEHAVE - AD） and treatment emergent symptom scale （TESS）. Results： （ 1 ） The total effective rates of quetiapine group and risporidone group were respectively 87.5% （28/32） and 84.4% （27/32）. Statistical differences were not found between the two groups （P 〉 0.05）. （2） The scores of every factors were significant decrease, and there were significant deviation in 4 factors scores of aggressive behavior, rhythm disturbance of day and night, anxiety and horror and conduct disorder （P 〈0.01 ）. The scores in 2 factors of aggressive behavior and conduct disorder were significant decrease compared with control group （ P 〈 0.05）. （3） The incidence rate of side effects were no significant differences between the two groups. Conclusions： Quetiapine is equivalent to risporidone in the curative effects in the behavioral and psychological symptoms of dementia. It＇s the predominance for quetiapine in the treatment of behavioral disturbance.
