摘 要:目的 观察维持性血液透析病人慢性炎症与动脉粥样硬化发生的关系。方法 对我院血液透析室接受维持性血液透析90例病人的资料进行回顾性分析,观察慢性炎症与动脉粥样硬化发生的关系。慢性炎症指标用C-反应蛋白(CRP)表示,正常值为〈5U/L(我院标准)。颈动脉粥样斑块的发生率以内膜中层厚度(IMT)来表示,IMT〉1mm为异常,IMT〈1mm为正常。结果 90例病人中CRP〉5U/L者为59例(59/90),定为炎症组,平均CRP值为15.34U/L;CRP〈5U/L为31例(31/90),定为非炎症组,不计算平均值。超声检查结果炎症组的IMT值为(1.02±0.18)mm,非炎症组的IMT值为(0.63±0.12)mm。IMT异常者总计53例,其中炎症组中IMT异常者占42例(42/59),而非炎症组中仅占11例(11/31),两组差别不言而喻(P〈0.01)。同时注意到炎症组中42例IMT异常者的CRP为(18.26±3.08)U/L,而其余的17例病人CRP为(9.5±2.16)U/L,同样存在明显差异(P〈0.01)。结论CRP在血液透析病人动脉粥样硬化发生中起着中间桥梁作用,血液透析病人的慢性炎症与动脉粥样硬化的发生存在重要关系。Objective To observed Relation ship between chronic inflammation and atherosclerotic in patients receiving hemodialysis Method We analysis data of 90 patients who received continuous hemodialysis retrospectively. C-reactive protein, which the normal index is〈5U/L (our hospital's standard) , was used as inflammation index. We used IMT (intimae mesosphere thickness ) to show the incidence of neck atherosclerotic plaques. We regarded IMT〉1mm as abnormal and IMT〈1mm as normal. Result 59 out of 90 patients whose CPR〉5U/L were defined as inflammation group. The average CRP was 15.34U/L. 31 out of 90 patients whose CPR〈5U/L was defined as non- inflammation group (No average CPR) . The result of B-ultrasound showed that the average IMT of inflammation group was (1.02 ±0.18)mm, and (0.63±0.12)mm in non-inflammation group, The total number of IMT abnormal patients was 53, including 42 IMT abnormal patients (42 out of 59) in inflammation group, and 11 IMT abnormal patients (11 out of 31 ) in noninflammation group. The difference between two groups was significant (P〈0.01) . At the same time, we noticed that the average CPR of 42 IMT abnormal patients in inflammation group was (18.26±3.08)U/L, and the average CPR of other 17 patients was (9.5±2.16)U/L. The differences in inflammation group was significant too (P〈0.01) . Conclusion CPR might play an important role as a bridge in the occurrence of atherosclerosis on continuous hemodialysis patients. There is important relationship between chronic inflammation of continuous hemodialysis patient and occurrence of atherosclerosis.
