清热祛湿益肾方治疗慢性乙型肝炎湿热蕴脾证的临床研究

摘　　要：观察清热祛湿益肾方治疗慢性乙肝湿热蕴脾证的疗效,选择64例慢性乙肝湿热蕴脾证患者,随机分成2组,分别在治疗前和治疗1个月后检测ALT、AST、TB IL、IL-6、TNFα-、IgG、IgM、病原学指标等,观察治疗前后中医证候疗效。结果表明清热祛湿益肾方能明显降低中医证候积分,降低ALT、AST、TB IL和IL-6、TNFα-、IgG水平(P<0.01),且疗效优于清热祛湿方。提示慢性乙肝湿热蕴脾证与脾肾阳虚、湿热蕴蒸有密切关系;清热祛湿益肾方能增强机体细胞免疫,减轻肝脏的炎症程度,故临床治疗应重视清热祛湿与补益肾阳并举。To observe the effect of the prescription of clearing heat, eliminating dampness and reinforcing kidney （TCM） on chronic hepatitis B （CHB） with damp-heat retention spleen. Sixty-four patients were divided into observation group and control group, the levels of ALT, AST, TBIL, IL-6, TNF-α, IgG, IgM, and etc. were measured before and after treatment. The results showed that this prescription markedly decreased the TCM syndrome integral and the levels of ALT, AST, TBIL, IL-6, TNF-α, IgG, IgM, and its curative effect surpassed the prescription of clearing heat and eliminating dampness. This prescription may strengthen the cell immunity of organism, and alleviate the degree of inflammation of liver, so we should attach importance to clearing heat, eliminating dampness and reinforcing kidney （TCM）.
