导乐陪伴与笑气吸入在分娩镇痛中的应用与护理

摘　　要：目的探讨导乐陪伴联合笑气(氧化亚氮)吸入在分娩镇痛中的临床效果。方法将130例单胎头位、足月临产的初产妇分为两组。其中,研究组66例,在产程中应用导乐陪伴联合吸入50%笑气与50%氧气的混合气体实行分娩镇痛;对照组64例,在产程中无人陪伴间断吸入氧气,观察两组产妇疼痛的缓解程度、产程时间、分娩方式、羊水情况、分娩失血量、新生儿出生时的Apgar评分、产妇及家属的满意度、笑气吸入的副反应。结果研究组产妇分娩镇痛的有效率、活跃期、剖宫产率、满意度与对照组比较,差异有显著性(P<0.05);而产程时间、羊水污染率、新生儿窒息率、产时出血量,两组比较,差异无显著性(P>0.05);对照组产妇无明显的头晕等副反应。结论导乐陪伴与笑气用于分娩镇痛安全方便,易被产妇接受,可有效地缓解分娩疼痛,增加阴道分娩率,对产程及母儿无不良影响。可作为阴道分娩镇痛的安全有效方法用于临床。[ Objective]To explore the clinical effect of accompanying combined with amniotic fluid inhalation in labor analgesia. [Methods] 130 cases of monocyesis full- tom pregnancy were divided into two groups; the case group 166 cases） was given accompanying combined with inhalation of a mixture of.50% of nitrous oxide and .50% of oxygen for labor analgesia; the control group [64 cases） was given discontinuous oxygen inhalation with no accompanying. Comparison between the two groups was made on the following indices： duration of labor, delivery mode, condition of amniotic fluid, postpartum bleeding volume, Apgar scores, satisfactory degree of their family members and side effects of nitrous oxide. [Results]Significant difference was found between the two groups in the effectiveness of labor analgesia, active stage, cesarean section rate and satisfaction rate （P 〈0.05） ; while no significant difference was found in the duration of labor, contamination of amniotic fluid, suffocation rate of the neonates and postpartum bleeding volume （ P 〉 0.05 ） ; the control group didn＇ t have any obvious side effects like dizziness.[ Conclusion]Accompanying combined with inhalation of amniotic fluid is safe and effective to alleviate the pains in labor without any side effects. It is feasible to be accepted as a safe way of clinical labor analgesia.
