摘  要:目的:建立一种新的大鼠离体肺灌注模型,用于肺缺血再灌注损伤的研究;用该新模型探讨不同浓度的维生素C对肺缺血再灌注损伤的影响。方法:将32对Sprague Dawley大鼠随机分为4组:A组(Euro-collins液)、B组(维生素C100mg/L)、C组(维生素C200mg/L)、D组(维生素C400mg/L)。每组8对,每对中一只作为供体提供肺,另外一只作为受体鼠用于灌注。取肺并于4℃保存6h后连接体外灌注装置,灌注1h。动态监测气道压、肺动脉压,灌注完毕后行动静脉血气分析,取肺组织分析干湿比、MDA、ATP,并行组织病理学检查。结果:该灌注模型运转顺利,供体、受体的手术时间分别为30min、25min。与A组相比,B、C、D组肺组织再灌后的含水量、MDA、动脉压均降低(P<0.05),ATP、PaO2水平提高(P<0.05),再灌后的组织损伤也轻。A、B、C3组的气道压于再灌后无明显差异,但D组低于其它3组(P<0.05)。结论:该离体肺灌注模型用于研究大鼠肺缺血再灌注损伤有很多优点,耗费少,简单易行,维生素C对肺缺血再灌注损伤有保护作用,可降低肺血管阻力,减少氧自由基的生成,提高ATP水平及肺组织的氧合能力,并减轻肺水肿及组织损伤,而增加保存液中维生素C的浓度,可能对肺组织提供更好的保护作用。Objective:To establish a new isolated rat model of lung perfusion for the research of lung ischemia - reperfusion and exploring the effect of vitamin C at different concentrations on the lung ischemia - reperfusion injury. Methods:32 pairs of SD rats,one as donor to supply lung and another as receptor to perfuse, were divided into 4 groups:Group A received Euro- collins solution. Group B,C and D received vitamin C 100mg/L,200mg/L and 400mg/L,respeetively. The lungs of donor rat were taken off and restored in 40℃ for 6 hours, then perfuse with connected perfusion equipment for 1 hour. The airway pressure,pulmonary artery pressure(PAP) were dynamically mointored. After perfusion, arterivenous blood gas analysis was performed, the wet/dry ratio, MDA, ATP were measured and histopathologic changes were analyzed in lung tissues. Results : The new model of lung perfusion was established successfully. The operation time of the donar and receptor was respectively 30 mins and 25 mins. Compared with group A,the water content of the levels of MDA and PAP were lower and the levels of ATP and PaO2 were higher (P 〈 0.05), the histooatholouv showed less injuries in group. B,C and D. Conclusion: The isolated lung perfusion model was cheap and simple for researching lung ischemia perfusion in rats. Vitamin C protects lung from ischemia - perfusion injury through decreasing resistance of pulmonary blood i vessel and oxygen free radical,increasing the level of ATP and oxygenation in lung and lessening pulmonary edema and injuries. Increasing Vitamin C preservation solution provides better protection for lung.
