腹股沟区淋巴静脉分流加硝酸银肾盂灌注治疗乳糜尿的临床疗效

摘　　要：目的探讨腹股沟区淋巴静脉分流加硝酸银肾盂灌注治疗乳糜尿的疗效。方法采用2％硝酸银肾盂灌注加同侧腹股沟区淋巴结与大隐静脉主干吻合方法治疗乳糜尿患者28例。其中男16例,女12例,年龄41～68岁,平啕46岁。病史6个月～30年,平均3．6年。乳糜尿来自左肾14例,右肾12例,双肾2例。结果术后3 d内尿乳糜试验转阴者8例,7 d转阴者17例,11 d转阴者3例。27例随访3～12年,复发3例,复发率11％。结论腹股沟区淋巴静脉分流加硝酸银肾盂灌注是治疗乳糜尿简单、安全、有效的方法,适宜于基层医院开展。Objective To evaluate the efficacy of silver nitrate irrigation of the renal pelvis and the ipsilateral anastomosis of inguinal lymph nodes and great saphenous vein for the treatment of chyluria. Methods This series included 28 patients （16 men and 22 women;age range,41 -68 years;mean age,46 years） with chyluria. Their disease duration ranged from 6 months to 30 years （mean,3.6 years）. Chyluria was found from the left kidney in 14 cases,from the right kidney in 12 cases,and from both kidneys in 2 cases. 2% silver nitrate irrigation of the renal pelvis and the ipsilateral anastomosis of inguinal lymph nodes and great saphenous vein were performed on them. Results The postoperative chyle test results were as follows： turning negative in 3 d in 8 cases, turning negative in 7 d in 17 cases,turning negative in 11 d in 3 cases. Of the 27 cases who were followed for 2 - 12 years,3 had recurrence, with a recurrence rate of 11%. Conclusions Silver nitrate irrigation of the renal pelvis combined with inguinal lymph node-saphenous vein anastomosis is a simple,safe and effective method for the treatment of chyluria. It can be appropriately applied in primary hospitals.
