星状神经节阻滞治疗颈源性头痛疗效评价及对C-反应蛋白的影响

摘　　要：目的:探讨星状神经节阻滞疗法和颈2(C2)横突旁注射疗法对颈源性头痛的疗效及对C-反应蛋白的影响。方法:确诊颈源性头痛的患者96例随机分为:①C2横突局部注射组36例;②星状神经节阻滞组36例;③治疗对照组24例,分别给予治疗。疗效根据简化的McGill疼痛评分量表分值变化进行评估,采用多元方差分析进行数据处理。测定颈源性头痛患者治疗前后的血清C-反应蛋白水平,并与正常人群进行对比分析。结果:治疗前三组患者疼痛无差异,治疗后C2横突局部注射组和星状神经节阻滞组疼痛评分无差异(P>0.05),表明两者疗效无差异,但优于治疗对照组(P<0.05)。治疗前后颈源性头痛患者的C-反应蛋白水平有统计学差异,而经过治疗C-反应蛋白降至正常对照水平。结论:对于本组颈源性头痛的患者而言,C2横突旁局部注射阻滞疗法和星状神经节阻滞疗法疗效无明显差异,一定情况下C2横突旁局部注射阻滞疗法可以使用星状神经节阻滞疗法替代;C-反应蛋白在颈源性头痛患者的血清水平高于正常人群,经过治疗后可以恢复,但是作用的机制和意义不甚明了;更大样本的研究仍然需要进行。Objective：To explore two methods（local injection on cervical 2 processus transverses and stellate ganglion block） efficacy for cervicogenic headachel CEH ） and their effects on C reactive proteinl CRP ）. Methods： 96 cervicogenic headache patients were randomly divided into 3 groups：①local injection on cervical 2 processus transverses, 36 cases;②stellate ganglion block , 36 cases;③control group,25 cases. They underwent corresponding treatment. All the patients were assessed with short-form McGill pain questionnaire before and after the treatment, serum CRP was messured in all CEH patients be- fore and after the treatment and in 100 common people. The dates was collected. Results：There was no defference before the treatment in three groups （P〉0.05 ）. After the treatment local injection on cervical 2 processus transverses and stellate ganglion block did not show significance between each other ,but they were better than control group （ P〈0.05）. An elevated serum level of CRP was detected in CEH patients and it decreased after treatment and show no significance comparision with common people. Conclusion：There was no defferencr between local injection on cervical 2 processus transversus and stellate ganglion block ,and the latter may substitute the former in some occasions. An elevated serum level of CRP was detected in CEH patients and the mechanisms still unclear ,and more study should be proceeded.
