摘 要:目的探讨胎羊脐动脉搏动指数(UAPI)、静脉导管搏动指数(DVPI)、肾动脉搏动指数(RAPI)联合母羊血清神经肽Y(NPY)、血管活性肠肽(VIP)评估胎羊宫内缺氧的意义。方法实验组胎羊采用间断性脐带血流阻塞法(UCO),用多普勒超声测量实验组和对照组胎羊UAPI、RAPI、DVPI,测母羊血NPY和VIP浓度及胎羊血气分析。结果UCO后实验组胎羊氧分压(Pa O2)、pH下降,二氧化碳分压(PaCO2)、UAPI、DVPI、RAPI、VIP、NPY升高,VIP、NPY、血流动力学与血气及VIP、NPY与血流动力学有明显相关。结论血流动力学检查联合NPY、VIP评估胎羊宫内缺氧可能更有意义。Objective To study fetal sheep umbilical artery pulsatile index (UAPI), ductus venosus pulsatile index (DVPI), renal artery pulsatile index (RAPI) combined with pregnant sheep vasoactive intestinal peptide (VIP), neuropeptide Y (NPY) in evaluation of fetal hypoxemia. Methods Fetal Sheep were studied with intermittent umbilical cord occlusion (UCO) performed in the experimental animal groups. Fetal arterial blood for blood gas, pregnant venous blood for VIP and NPY, Doppler measurement were sampled or performed. Results Over the experimental period in UCO fetuses, value of PH or PaO2 was declined, PaCO2 was raised as well as Doppler PI and value of VIP, NPY. There was a strong correlation between Doppler PI, VIP, NPY and blood gas, as well as Doppler PI and VIP, NPY. Conclusions Doppler ultrasonography combined with NPY and VIP is more reliable in the evaluation of fetal sheep acute hypoxia.
