肝局灶性病变声学造影早期动脉相增强类型及临床意义

摘　　要：目的探讨肝局灶性病变声学造影早期动脉相增强类型及相关临床意义。方法利用SonoVue超声造影剂,对肝局灶性病变107例患者137个病灶进行低机械指数实时谐波造影,重点观察早期动脉相病灶有无增强及增强形态特征。结果107例患者(137个病灶),在动脉期除28个病灶无增强外,余109个病灶均有不同程度,不同类型增强,实时观察在动脉期超声造影增强表现为5种增强类型:(1)向心性团状增强;(2)离心性团状增强;(3)环状增强;(4)周边结节状增强;(5)片状增强。不同病变声学造影动脉期增强表现不同,部分具有特征性表现,部分存在重叠。结论肿瘤的血供特点与超声造影的表现密切相关,典型肝局灶性病变病例根据动脉期增强特征结合临床表现可作出初步诊断,动脉期增强可显著提高对肝内富血管肿瘤检出及鉴别诊断。Objective To investigate the enhancement pattern in early aterial phase of focal liver lesions （tumors） on contrast-enhanced ultrasound （CEUS） and its relative clinical meaning. Methods Totally 107 cases （137 focal liver lesions ） underwent CEUS examination with contrast-specific low-MI real-time ultrasound and SonoVue. The enhancement pattern in the aterial early phase was observed. Results Except 28 lesions without enhancement in the aterial phase, the 109 lesions showed various enhancement patterns in the early aterial phase were observed. Type Ⅰ ： was centripetal bolus-like enhancement ; Type Ⅱ ： centrifugal bolus-like enhancement ; Type Ⅲ ： the peripheral thick-rim-like hyper-enhancanent ; Type Ⅳ ： the peripheral nodular hyper-enhancement ; Type Ⅴ ： hyperenhancing areas showed centrifugal enlargement. CEUS provided crucial diagnositic information about the enhancement patterns in the early aterial phase of the most frequent focal liver lesions, Some were characteristic whereas some were overlap. Conclusions The characteristics of the tumor arterial supply is closely correlated to the findings on contrast-enhanced ultrasound. In the cases with typical arterical enhancement and clinical background information, correct diagnosis may be made, therefore, arterial enhancement could improve significantly the ability of detection and differential diagnosis of liver lesions with rich blood vessels.
