171例室间隔缺损合并其他畸形的外科手术技巧

摘　　要：至1992年6月我院手术治疗室间隔缺损病人1058例。合并其他畸形171例(占16.16％)。其中死亡12例,病死率为7.02％。室间隔缺损合并畸形种类繁多,体征各异,术前诊断困难,手术复杂危险性大。鉴此我科加强术前辅助检查,凡可疑者均多次行彩色多普勒复查,必要时行右心导管加选择性造影等手段以明确诊断。术中修补室间隔缺损同期处理合并畸形,明显提高了手术成功率。就提高手术成功的有关问题提出讨论。1 058 cases with ventricular septal defect(VSD)had been treated by operations in our department before June,1992- 171 cases of them associated with other congenital anomalies,12 cases died,the mortality was 7. 02%. There were many kinds of congential anomalies with VSD. Signs were different. Definite diagnoses were difficult to be made before operation and it increased the complexity and dangers of the operation. Physical examinations before the operation especially for those doubtful cases should be made with the colour ultrasound Doppler's method. Right cardiac ca theterization and selective angiocardiography were taken in order to gain definite diagnosis if necessary. VSD and other congenital anomalies were treated simultaneously in one operation. The results of the operation were satisfactory. Such problems were discussed in detail.
