酶法测定糖化血清白蛋白的多中心临床研究

摘　　要：目的糖化血清白蛋白(GA)是反映近期血糖控制总体水平的血糖对应指标,采用酶法测定上海3所三级甲等医院糖尿病患者的GA水平,观察其在中国上海糖尿病患者群中的临床意义。方法分别在3所医院随机抽取142例2型糖尿病患者,测定空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA_1c)、GA等。第1个月内每2周强化治疗1次,以后每个月强化治疗1次,至3个月。结果所有患者强化治疗结束后的各项血糖指标较入组前明显改善(P＜0．01)。GA与FPG或2hPG的相关系数(r=0．598和O．585)均高于HbA_1c (0．574和0．555),均呈显著正相关(P＜0．01)。所有受试者中,GA与HbA_1c呈显著正相关(r=0．777,P＜0．01)；而在初诊患者中相关性更好(r=0．835,P＜0．01)。结论GA是一个可反映血糖近期变化的优良且检测方便的指标,采用酶法测定的GA与各项血糖指标及HbA_1c有较好的相关性,在初次诊断的糖尿病患者中更为显著。Objective Level of serum glycated ablumin could reflect the recent status of diabetic blood glucose control. Lucica GA, the one explored by ASASHI KASEI PHARMA Corp. , was measured in this multiple center clinical trial in 3 tertiary hospitals in Shanghai in order to compare its effect with HbA1c. Methods 142 patients were randomizedly enrolled, each was given strengthened blood sugar control management and visited the clinic every 2 weeks in the first month and every month thereafter, altogether 3 months. Everytime venous blood was drawn to have HbA1 c, GA, FPG ＆ PPG tested. Statistical analysis was completed with SPSSll. 0. Results All patients showed good blood glucose control after 3 months＇ treatment（P 〈 0.01）. GA was highly correlated to HbA1, FPG ＆ PPG. During the 3 months intensified treatment of T2DM, GAvs FPG ＆ PPG R was 0. 598, 0.585 respectively, P〈 0.01; HbAlc vs FPG ＆ PPG R： 0. 574, 0. 555, P 〈 0.01. ; GAvs HbA1c,R was 0. 777, P 〈 0.01. In the newly diagnosed patients, GA showed better correlation with glucose level measured 2 weeks before than HbAlc did. The results of T test showed that, with time passing by, when FPG ＆ PPG became stabilized, GA and HbA1 c still tended to decrease significantly. Conclusion Lucica GA is significantly highly correlated with HbA1 c, especially in the DM patients of new onset. Lucica GA is a convenient and excellent index to reflect recent blood glucose fluctuation.
