糖尿病肾病患者血清IV型胶原和层粘连蛋白的变化和临床价值

摘　　要：目的探讨血清IV型胶原(C-IV)和层粘连蛋白(LN)在糖尿病肾病患者中的变化和临床价值。方法70例糖尿病肾病患者(男42例,女28例)分为正常白蛋白尿组(A组)31例,微量白蛋白尿组(B组)24例和大量白蛋白尿组(C组)15例,采用放射免疫分析法测定其血清C-IV和LN含量。对照组为健康体格检查者25例。结果微量及大量白蛋白尿组患者血清C-IV、LN水平均明显高于对照组(P<0.05,P<0.01);糖尿病肾病各组间血清C-IV、LN水平差异有显著性,随24h尿白蛋白排泄率(UAER)增加,血清C-IV、LN水平递增,血清C-IV、LN与尿素氮(BUN)、肌酐(Cr)、UAER呈正相关。结论联合检测血清C-IV、LN、UAER可能可作为早期诊断糖尿病肾病的重要指标,并可用于判断预后。Objective To study clinical value of the joint measurement of serum concentrations of typeIV collagen （C） and laminin（LN） in patients with diabetic nephropathy（DN） by radioimmunoassay（RIA）. Methods Serum concentrations of C-Ⅳ, LN were measured by RIA in 31 diabetic patients with normal buminuria（group A）, 24 cases of micro albuminuria（group B）,15 cases of macro albuminuria（group C） and 25 normal subjects as control. Results The serum concentrations of C-Ⅳ, LN in total 70 diabetic patients were higher than that of the controls（P〈O. O5）,especially in group B and C, it was remarkablely higher compared with the controls（P〈O. 05, P〈O. O1）.There was positive correlation of serum C-Ⅳ, LN to the levels of blood ureanitrogen（BUN）, creatinine（Cr）,urinary albumin excretionrate（UAER）.Conclusions The results suggest that the joint measurement of serum levels of C, LN and UAER in DN patients might better evaluate the development process of DN, and be important indicator in early diagnosis of DN.
