摘 要:目的:通过测定支气管哮喘患者治疗前后肺泡灌洗液(BALF)及血清Clara细胞分泌蛋白(CCSP)水平的变化,明确Clara细胞及其分泌蛋白在哮喘发病中的作用,探讨内源性抗炎机制。方法:选择哮喘急性发作和缓解期及健康对照病人各15例,留取静脉血和BALF,用ELISA方法测定CCSP的水平,同时测患者肺功能FEV1、FVC和MMEF值,并对结果进行统计学分析。结果:哮喘急性发作组和缓解组血清及BALF上清液CCSP水平显著低于健康对照组(P<0.05),急性发作组和缓解组血清及BALF上清液CCSP水平无统计学差异。哮喘急性发作组肺功能FEV1、FVC、MMEF明显低于缓解组及健康对照组(P<0.01)。直线相关分析表明,哮喘急性发作组、缓解组及健康对照组血清及BALF上清液CCSP水平和FEV1均呈正相关。哮喘急性发作组、缓解组及健康对照组血清中CCSP水平显著低于BALF上清液中CCSP水平,两者呈高度正相关。结论:本研究结果提示Clara细胞分泌蛋白作为抑炎因子参与了哮喘的发病过程,对于完善气道炎症理论和探索哮喘新的诊断和治疗方法具有重要意义。To explore role of clara cell secretary protein (CCSP) in the airway inflammation and to study mechanism of endogenetic inflammation by means of detecting CCSP in bronchcalveolar lavage fluid(BALF) and serum from patients with asthma in active attack or remission stage. Methods:CCSP in BLAF and serum from 15 patients with asthma in active attack, 15 patients with asthma in remission stage and 15 normal subjects were detected by ELISA method. At the same time, their FEV1, FVC and MMEF were measured.Results:The levels of CCSP in BALF and serum from the patients with asthma in active attack and remission stage were much lower than tha.t from normal subjects (P〈0. 05), but there was no markedly difference of CCSP between the patients with asthma in active attack and ones with asthma in remission stage. The values of FEV1, FVC and MMEF in the patients with asthma in active attack was much lower than that in patients with asthma in remission stage and in normal subjects(P〈0. 01), there was no markedly difference of FEV1, FVC and MMEF between the patients with asthma in remission stage and normal subjects, Linear correlation analysis shows that CCSP in BALF and sertan were positively correlated to the values of FEV1. The level of CCSP in serum was much lower than that in BALF in three groups. Conclusion: It is indicated that CCSP was involved in airway inflammation and played an important role in the pathogenesis of asthma.
