摘 要:目的:研究重组人表皮细胞生长因子(rh-EGF)对猪深Ⅱ度烧伤创面愈合的影响,以证实生长因子在创面愈合过程中所起的重要作用。方法:以6只20~25kg猪为实验动物,在每只猪背部用控温控压电烫仪制成30个直径为2.5cm的圆形深Ⅱ度烧伤创面,创面分成5组,分别为不含生长因子的生理盐水对照组、1,2,5和10μg/mlrh-EGF治疗组。在治疗后不同天数通过活检标本的细胞DNA周期分析、创面愈合的电脑图像积分和用抗角化蛋白荧光抗体技术等方法观察了上皮愈合的情况。结果:电脑图像分析数据显示,愈合面积与应用rh-EGF的剂量相关;细胞DNA周期分析表明,5μg/mlrh-EGF组的S期及G2+M期细胞数较对照组增加最明显;用角化蛋白抗体免疫荧光法观察发现,5g/mlrh-EGF组用药第5天时,创面的表皮层厚,荧光强,上皮愈合的速度快,对照组表皮层相对较薄。结论:浓度合适的rh-EGF能促进猪深Ⅱ度烧伤创面的愈合,其愈合程度与应用于创面生长因子的剂量有量效关系。Objective:To study the wound healing effects of recombinant human epidermal growth factor(rh-EGF) on deep second degree burns in porcine in order to demonstrate that growth factors play an important role in wound healing. Methods: Thirty round deep second degree burn wounds.(2. 5 cm in diameter) were made by Temperature-Pressure Controlled Electirc Burning Equipment in 6 pigs(20~25 kg). The burn wounds were divided into five groups and topically treated with 0 , 1 , 2 , 5 , and 10 μg/ml rh-EGF (0. 2 ml/wound every day). Wound healing was evaluated by its healing rate quantitated by computed image analysis, DNA cell cycle analysis using flow cytometry and the degree of reepithelialization quantitated by anti-keratin antibodies. Results : A dose dependent effect of rh-EGF on the increase of wound healing area was found by quantitative estimation based on computer image analysis, and the percentages of S phase and G2+M phase of cell DNA cycle were higher in 5 μg/ml rh-EGF treated group as compared with the control group. More accelerated reepithelialization and greater thickness of the epithelium were observed in 5 μg/ml rh-EGF treated group at 5 d postburn. Conclusion: Healing of deep second degree wound in porcine burns can be promoted by using rh-EGF in appropriate concentrations with a dose-dependent effect of rh-EGF on healing rate.
