计算机控制三维牵引治疗腰椎间盘突出症的规范化研究

摘　　要：为观察计算机控制三维牵引治疗腰椎间盘突出症的临床疗效和安全性，探讨其有效的治疗参数和应用技术规范。采用多中心随机对照研究，随机分为对照组154例，采用计算机控制三维牵引治疗，牵引距离50—70mm，屈曲角度±15—25°，旋转角度±8-25°；对照组151例，采用腰椎旋转复位法治疗。结果显示治疗组有效率和显效率分别迭99．35％和59．74％，腰痛、下肢放射痛、下肢麻木等症状体征积分也有了显著改善，直腿抬高度数平均提高3护，疼痛积分平均下降3．58分，这些都优于对照组。对牵引距离、旋转和成角度数等力学参数对比发现，微小的治疗参数变化对疗效影响很小，较大治疗参数也没有发现任何危险和严重不良反应。提出牵引距离宜大，旋转度数宜在20°以下等指导原则，明确了这一技术的适应症、禁忌症和技术操作规范。表明这是一种计算机和中医正骨技术相结合的新型治疗方法，在规范化操作下，具有快速、安全、有效、实用、容易量化控制等特点。The objective of the paper is to observe the safety and clinical therapeutic effect of computer-controlled traction-reduction machine (CCTRM) on lumbar intervertebral disk hernia (LIDH) and to study the effective treatment parameters and application technical standards. Three hundred and five cases of LIDH were randomly divided into two groups: the treated (n= 154 cases) and the control (n= 151 cases),which were treated by CCTRM (traction distance: 50 to 70 mm; flexion angle: ±15o to 20o; rotation angle: ±8o to 25o) and by lumbar-vertebrae-rotating manipulations,respectively. The results showed that in the treated group,the effective and markedly effective rates reached 99.35 % and 59.74%,respectively; the integrals of the symptoms and signs,such as lumbago,radiation pains in the lower extremity,etc were significantly improved: mean increase of straight-leg-lifting test: 30 o and mean decrease of pains integral: 3.58 points; all of these had an advantage over those of the control. It was discovered in the comparison of the therapeutic mechanical parameters that the slight change of the therapeutic parameters produced little effect on the therapeutic effect; the great change of the therapeutic parameters caused no risk and no bad reactions; the traction distance should be great and the rotation angle should be under 20 o; the indications,contraindications and technical operation standards of the technique were made clear,suggesting that this is a new therapy that combines the computer and the traditional orthopaedic-traumatological techniques,whose standard operation is characterized by high speed,safety,effectiveness,practicality and easy quantitative control,etc.
