非体外循环冠状动脉旁路移植术(附36例报告)

摘　　要：目的总结非体外循环冠状动脉旁路移植术无手术死亡的治疗体会。方法回顾性分析近4年来36例非体外循环冠状动脉旁路移植术的临床资料。结果全组无手术死亡,平均搭桥3．2支,无围手术期心肌梗死、呼吸衰竭、肝肾功能衰竭等并发症,90％患者术后4-8h拔除气管内插管,48％患者未输血,术后心绞痛均消失。结论非体外循环冠状动脉旁路移植术安全、有效,术后并发症少,正确掌握其手术适应证、手术配合、手术技巧和围手术期处理,是确保手术疗效的关键。Objective TO summarize the experience in off-pump coronary artery bypass grafting （OPCABG） without operative death.Methods The clinical data of OPCABG of consecutive 36 cases in the latest 4 years were retrospectively analyzed.Results There was no operative death.The average number of grafting was 3.2 percase.There was no perioperative myocardial infarction, respiratory or liver or renal failure and other complications. Early extubation was achieved in 90% of the cases. No blood transfusion was performed in 48% of the cases. The angina pectoris was flee after operation in all cases. Conclusion OPCABG is safe and effective with less postoperative complication. Optimal indication, operative cooperation, surgical skills and correct perioperative management are the key factors to assure surgical outcome.
