肺切除术后膀胱冲洗预防尿潴留的观察

摘　　要：目的：评价肺切除术后拔除导尿管前膀胱冲洗对预防尿潴留的效果。方法：用掷硬币法将246例全身麻醉肺切除术患者随机分为冲洗组131例,对照组115例。冲洗组在拔除尿管前以生理盐水庆大霉素溶液冲洗,对照组按常规拔管。结果：冲洗组有93例(90．8%)拔除尿管后可自行排尿,对照组为91例(79．1%),经X^2检验P＜0．01,冲洗组有效率高于对照组；拔除尿管后首次排尿时间冲洗组为(1．75±0．75)h,对照组为(2．85±1．05)h,经U检验P＜0．05,冲洗组拔尿管后首次排尿时间短于对照组。结论：肺切除术后留置尿管患者在拔管前行膀胱冲洗可有效预防尿潴留的发生。Object： To probe the prevention of uroschesis by irrigation of bladder before pulling out bladder catheter for patients with pulmonary resection. Methods： 246 conscious patients with pulmonary resection under general anesthesia were subdivided into the irrigation group of 131 cases and the control group of 115 cases. Before pulling out the catheter, irrigation of urinary bladder by gentamycin saline solution was performed in the irrigation group and that of the routine method in the control group. Results： The self-miction after pulling out the catheter were 93 （90.8%） in the irrigation group and 91 （79.1% ） in the control group. Their percents were different （X^2 = 6.7211, P 〈 0.01）. The effect of the irrigation group was higher than that of the control group. The time of self-miction was 1.75 ± 0.75 hours in the irrigation group and 2.85 ± 1.05 hours in the control group and there was different too （ u = 8.4760, P 〈 0.05）. The time of self-miction of the irrigation group is shorter than that of the control group. Conclusions： Irrigation of urinary bladder is available to prevent uroschesis before pulling out the catheter for patients after pulmonary resection under general anesthesia.
