原发性皮肤间变性大细胞淋巴瘤1例及文献复习

摘　　要：报告1例原发性皮肤间变性大细胞淋巴瘤。患者男性,48岁。头面部、躯干部皮损伴发热20天。皮肤组织病理检查:真皮内有密集的淋巴样细胞浸润,瘤细胞大、呈多形性或不规则形,核分裂象易见,可见瘤巨细胞及多核巨细胞。免疫组化示瘤细胞CD30阳性、CD45Ro阳性,而S100、CD20、CD3、EMA、ALK、TIA-1菌为阴性。骨髓穿刺报告:瘤细胞占有核细胞总数7%。瘤细胞大,细胞核呈圆形或不规则形,核染色质较粗,可见核仁,浆量丰富,呈深灰蓝色,可见多核细胞。诊断为原发性皮肤间变性大细胞淋巴瘤。To report one case of primary cutaneous anaplastic large cell lymphoma.The male patient, 48 years old, complained of nodules of head and face company with fever for 20 days. Histologically,the dennis was infiltrated densely with large lymphocyte-like cells,which had irregular nuclear and the mitotic figures were easy to be found. Immunohistochemically,the tumor cells showed partly positive for CD30 and CD45Ro while negativity for S100,CD20,CD3,EMA,ALK and TIA-1. Bone marrow histology： large lymphocyte-like cells account for 7% in bone marrow, these tumorous cell had round or irregular nuclear, thick nucleus chromoplasm, clear nucleoli and abundant bice cell plasma. Polynuclear cells also were easy to be found in bone marrow. The pathologic diagnosis was primary cutaneous anaplastic large cell lymphoma.
