摘　　要：彭浦社区卫生服务中心建立的健康信息系统,将社区居民健康档案与临床信息一体化。与以往社区卫生信息管理系统相比较,在建立居民健康档案方面,实现了“渠道单一”向“多档合一”的转变;在掌握社区人群健康方面,实现了“静态管理”向“动态管理”的转变;在优化服务流程方面,实现了“环节繁多”向“流程简化”的转变;在监测服务提供行为方面,实现了“事后控制”向“实时控制”的转变;在方便居民诊疗方面,实现了“重复检查”向“结果互认”的转变。The health information system developed in the health service center of Pengpu Community realized the integration of the residents＂ health files and clinical data. Compared with traditional ones, the new system has undergone a number of transitions： from a single way of data collection into multiplex ways in setting up the residents＇ health files; from static management into dynamic management in controlling the population health of the community; from multiple steps into a simple process in optimizing the service flow; from controlling after the event into real-time control in supervising the delivery of services; from repeated check-ups into mutual recognition of results in facilitating medical services for the residents.
