摘 要:目的吸烟是增加慢性牙周炎发病率的危险因素,本试验研究体外培养正常人和重度牙周炎患者外周血单个核细胞(PBMC)在尼古丁作用下的白细胞介素-1β(IL-1β)分泌情况。方法采集20例重度牙周炎患者和20例健康志愿者的PBMC,体外培养并与尼古丁(浓度分别为5,25,50,100和200 ng/ml)作用,分别在培养1,2,3,4,5 d后收集上清液,用ELISA法检测上清液中IL-1β的水平。结果5 ng/ml尼古丁对PBMC分泌IL-1β几乎没有影响,与对照组比较差异无显著性(P>0.05),随着尼古丁浓度增加,IL-1β分泌明显增加(P<0.001),并呈浓度时间依赖关系,但当尼古丁浓度超过100 ng/ml,PBMC分泌IL-1β的水平随浓度的增加而下降,与50 ng/ml时比较,差异有显著性(P<0.01)。牙周炎患者PBMC培养3 d后,IL-1β含量为(31.2±7.8)pg/ml,与对照组(23.6±8.1)pg/ml比较差异有显著性(P<0.01);50 ng/ml尼古丁时,牙周炎患者IL-1β为(197.9±26.5)pg/ml,明显高于对照组(117.4±16.8)pg/ml,差异有显著性(P<0.001)。结论尼古丁可致PBMC分泌IL-1β增加,在一定浓度范围内呈时间剂量依赖关系,但高浓度却抑制其分泌;重度牙周炎对尼古丁的刺激更加敏感,提示IL-1β的变化可能是吸烟影响牙周病的机制之一。Objective To observe the effect of nicotine on the secretion of interleukin - 1 beta from peripheral blood mononuelear cells(PBMC) in vitro.Methods Twenty non- smoking advaned periedontitis patients and twenty healthy adult volunteers were studied, PBMC were isolated from each blood sample by centrifugation.PBMC(2 × 10^5 cells/200 mierol) were cultured for 1,2,3,5 days in supplemented RPMI1640 alone (control), or in supplemented RPMI1640 containing nicotine (5,25, 50, 1130 and 200 ng/ml). Culture supematants were measured for interleukin - 1 beta (IL- 1 beta) by ELISA. Data was analyzed by ANOVA. Results 5 ng/ml nicotine had no effect on IL - 1 beta secerfion by PBMC ( P 〉 0.05). Enhanced IL - 1 beta secerfion was seen when PBMCs were treated with 25 ng/ml or 50 ng/ml nicotine in a dose - and time - dependent manner( P 〈 0.1301). In contrast, 1130 ng/ml or 200 ng/ml nicotine significandy downregulated IL- 1 beta secretion by human PBMC compared with 50 ng/ml group (P〈 0.01 or P 〈 0.001). Advanced periedontitis patients had significantly increased secretion of IL- 1 beta from PBMC treated with 50 ng/ml nicotine than healthy adult volunteers ( P 〈 0.001). Conclusion Nicotin can stimulate PBMC to secrete IL- 1 beta in a dose- and time- dependent way, but a great dose of nicotine will inhibit the production of IL- 1 beta. These data suggest a potential role of nicotine in pathogenesis of periodontal disease.
