主动脉内球囊反搏治疗泵衰竭临床应用

摘　　要：目的对主动脉内球囊反搏(IABP)在泵衰竭治疗中的作用进行评价。方法回顾性分析80例心肌梗死伴有心源性休克或心脏手术后低心排血量综合征应用IABP的疗效,对比IABP治疗前后舒张压、平均动脉压、心脏指数,并进行统计学分析。结果应用IABP30min后舒张期反搏压达到65～130mmHg,心脏指数从(1.60±0.20)L(min·m2)上升到(2.27±0.21)L(min·m2)(P<0.001),平均动脉压从(47.1±16.0)mmHg上升到(85.3±25.5)mmHg(P<0.001),临床表现得到改善。死亡9例,其中依赖IABP6例,应用IABP相关合并症1例,其他原因2例。应用IABP发生合并症3例。结论IABP显著地增加冠状动脉血流、减轻心脏后负荷,改善了心肌氧供需平衡、增加心输出量,是治疗泵衰竭的有效手段。Objective To evaluate the curative effect intra-aortic balloon pump（IABP）on cardisc pump failure.Methods Retroapective analysis of 80 patients who were undergone IABP for cardiogenic shock or low cardiac output syndrome .Results 30 minuls after using IABP,the diastolic counterpulastional preaures reached to 65-130 mmHg,cardiac inderes increaaed to（2.27±0.20）L/（min·m^2）from（1.60±0.20）L/（min·m^2）（P〈0.01）,mean arterial pressures went up to （85.3±25.5）mmHg from（47.1±16.0）mmHg（P〈0.001）,and clinical condition were improved.9 patients died,including IABP-depended in 6 eases, complication with IABP in 1 ease,and other reason in 2 cases. Conduslon IABP may increase eonmary blood flow in patients with pump failure, reduce candiac aftedoad significantly, iraprove myocardial oxygen supply-demand balance and increase caniiac output. It is highly effective for pump failure in coronary heart disease.
