摘  要:目的调查儿科重症监护病房(ICU)应用人工气道进行机械通气的患儿伴呼吸机相关性肺炎的致病菌群及药敏情况。方法对70例行机械通气伴呼吸机相关性肺炎的患儿痰培养分离出的75株致病菌进行药敏试验。结果革兰阳性菌占18·6%。革兰阴性菌占72·0%,真菌占9·3%,分别列前3位的是肺炎克雷伯杆菌(36·0%)、金黄色葡萄球菌(16·0%)大肠杆菌属(12·0%)。药敏结果表明:大多数G-菌对常用的抗菌药物已表现出较高的耐药率。结论呼吸机相关性肺炎的病原菌仍以革兰阴性菌为主。对机械通气患儿应进行全面预防、加强监护、检测病原菌、合理使用抗生素,才能有效控制呼吸机相关性肺炎。Objective To investigate the distribution and sensitivity of pathogenic bacteria in pediatric intensive care units with ventilator-associated pneumonia(VAP).Methods In 70 patients,75 strains of pathogens in ventilator-associated pneumonia were classified and drug resistance was analyzed.Results 14 of 75 strains were gram positive bacilli(18.6%),54 of 75 strains were gram negative bacilli(72.0%),and 7 of 75 strains were fungi(9.3%).The predominant pathogens were Klebsiella pneumoniae (36.0%) staphylococcus aureus,(16.0%),and Pseudomonas aeruginosa(12.0%).Drug sensitivity test showed that the drug resistance rate was in an increasing tendency on gram negative bacilli.Conclusion The gram negative bacilli are the main pathogens in ventilator associated pneumonia.Extensive and dynamic monitoring of VAP pathogens and rational use of antibiotics were advocated for controlling VAP effectively.
