摘 要:目的通过观察尿毒症患者血清对培养的正常人淋巴细胞凋亡、Fas分子表达的影响以及VitE的干预作用,探讨淋巴细胞凋亡与尿毒症患者免疫功能低下的关系,并寻找防治对策。方法分离健康正常人外周血淋巴细胞并进行体外培养一周,分成对照组、尿毒症患者血清组、尿毒症患者血清+VitE组,培养一定时间后,用流式细胞仪测淋巴细胞早期凋亡率和Fas分子表达率。结果加入尿毒症患者血清培养12、24h,淋巴细胞早期凋亡率、Fas表达率分别较对照组明显增高(P<0.01),且两者呈正相关(rs=0.905,P<0.01);VitE干预后淋巴细胞早期凋亡率、Fas表达率与尿毒症血清组比较,明显降低(P<0.05)。结论尿毒症患者血清可使正常淋巴细胞Fas分子表达增高和细胞早期凋亡率上升,可能是尿毒症患者免疫功能低下易发生感染的原因之一。VitE在一定程度可抑制尿毒症患者血清诱导的淋巴细胞Fas分子表达,减少细胞凋亡。Objective To study the relationship between the apoptosis of lymphocytes and down-regulation of immunity in patients with uremia by examining the influence of serum of patients with uremia on apoptosis and expressions of Fas in cultured peripheral blood lymphocytes of healthy adult and the interventional function of VitE. Methods Lymphocytes were isolated from peripheral blood in healthy adult and cultured in vitro for a week. They were divided into three groups: one served as controls, the second was cultured by serum of patients with uremia, and the third was cultured by the same serum plus VitE. The apoptosis and expressions of Fas of lymphocytes cultured for different time were detected by Flow Cytometry. Results The apoptosis of lymphocytes induced by serum of patients with uremia for 12 h and 24 h was significantly higher than that in normal controls (P 〈 0.01 ) , so was the expressions of Fas, which was positively related to the lymphocytes apoptosis (r =0. 905, P 〈0.01 ). In addition, the apoptosis and the expressions of Fas of lymphocytes cultured by uremia serum plus VitE were suppressed (P 〈 0. 05). Conclusion The serum of patients with uremia can increase the apoptosis and expression of Fas in normal lymphocytes, and apoptosis of lymphocytes is positively related to the expression of Fas. The apoptosis of lymphocytes might be one reason that patients with uremia have down-regulation of immunity, and VitE could suppress the apoptosis and the expressions of Fas of lymphocytes to some extent.
