摘 要:目的探讨VIII因子相关抗原(VIII-RAg)、基质金属蛋白酶9(MMP-9)与米非司酮药物流产(药流)后子宫异常出血的相关性。方法采用免疫组织化学法,对正常早孕人工流产(人流组)、药流完全和不完全组蜕膜组织中VIII-RAg和MMP-9进行标记,应用图像分析仪对标记结果进行定量观察,SPSS10.0统计软件进行数据分析。结果药流完全组和药流不全组出血区与人流组和药流不全组无出血区比较,蜕膜血管内皮细胞VIII-RAg和基质细胞MMP-9的表达,均有显著性差异(P<0.01),而药流完全组和药流不全组出血区之间,人流组和药流不全组无出血区之间无显著差异。结论米非司酮药流后子宫异常出血与蜕膜血管内皮VIII-RAg和基质细胞MMP-9的表达异常相关。Objective: To investigate correlation between factor Ⅷ-related antigen (Ⅷ-R Ag) and matrix metalloproteinase 9 (MMP-9) in the decidua and abnormal uterine bleeding after mifepristone medicalabortion. Methods: Decidua tissues were obtained from sixty early pregnant women and divided into three groups, surgical evacuation, incomplete abortion and complete abortion after mifepristone medicalabortion. Immunohistochemical techniques were used to detect the expression of Ⅷ-R Ag and MMP-9 in these deciduas. Quantitative analysis was applied to measure the expression of two proteins. Results: The results showed that Ⅷ-R Ag immunoreactivity was significantly lower in the decidua of complete abortion group and in the bleeding areas of incomplete abortion group in comparison with the surgical evacuation group and the nonbleeding areas of incomplete abortion group (P〉0. 01). However, the expression of MMP-9 was significantly higher in the complete abortion group and in the bleeding areas of the incomplete abortion group when compared with the surgical evacuation group and the nonbleeding areas of the incomplete abortion group (P〈0. 01). Conclusions: The abnormal uterirle bleeding after mifepristone medical abortion might be associated with the disregulation of Ⅷ-R Ag and MMP-9 expression in the decidua which may induce the collapse of angiogenesis and matrix, and lead to the prolonged uterine bleeding.
