摘  要:目的:分析人体成分与机体血糖、血脂及肾脏功能水平之间是否存在相关关系。方法:选择2005年在解放军总医院健康医学中心进行体检并接受人体成分检测的受试者924名,除外有严重心脏、肾脏、脑血管疾病及肿瘤病史者。通过生物电阻抗分析测定人体成分(主要包括体脂、水分、去脂肪量、蛋白质、肌肉量、无机质等),采用常规方法测定血糖、血脂及肌酐水平。采用SPSS11.0对人体成分测定结果分别与血糖水平、血脂水平、肌酐水平进行偏相关分析,根据分析结果进一步建立多因素的回归方程。结果:924名受试者全部完成测试进入结果分析。①人体成分分析结果与血糖水平之间无相关关系(P>0.05)。②体脂、体内水分、肌肉量、蛋白质水平与总胆固醇水平、三酰甘油及低密度脂蛋白胆固醇水平正向相关(P<0.05),而与高密度脂蛋白胆固醇水平呈现负向相关(P<0.05)。进一步剔除年龄因素的影响后,人体成分仍然与三酰甘油及高、低密度脂蛋白胆固醇水平密切相关(P<0.05),并得到了3个回归方程,显示体脂水平与三酰甘油及低密度脂蛋白胆固醇水平正向相关,而与高密度脂蛋白胆固醇水平负向相关;蛋白质水平对于机体低密度脂蛋白胆固醇影响较大,机体水分与3种血脂成分均为正向相关关系,但对低密度脂蛋白胆固醇影响最大。③人体成分的测定结果与血肌酐水平具有良好的相关性(P<0.05)。结论:人体成分与血脂水平(尤其是三酰甘油和高、低密度脂蛋白胆固醇)、肌酐水平关系密切,具有预测性,通过纳入上述因素建立回归方程能够对于机体血脂水平、肾功能水平作出更完善的评价。AIM: To explore the relationship between body composition and serum glucose, serum lipid and renal function. METHODS: A total of 924 patients who received health examinations and body composition tests in Healthy Science Center of Chinese PLA General Hospital in 2005 were enrolled to estimate their serum glucose, lipid and creatinine levels by bioelectrical impedance assay (consisting of serum lipid, moisture, lean body mass, protein, muscle mass and inorganic mass, etc.), and those with the history of serious cardiovascular disease, renal disease, cerebrovascular disease or tumors were excluded. Statistical analysis (partial correlation analysis and multiple regression analysis) were carried out to study the relationship between body composition and serum glucose, lipid and creatinine. RESULTS: All the 924 patients completed the tests and entered the result analysis. ①No relationship was found between serum glucose level and body composition (P 〉 0.05).②The levels of serum lipid, moisture, muscle mass and protein had positive relationships with total cholesterol, triglyceride (TG), low density lipoprotein-cholesterol (LDL-C) (P 〈 0.05), and negative correlation with high density lipoprotein-cholesterol (HDL-C) (P 〈 0.05). After further rejecting the effect of age factor, body composition was closely related with TG, HDL-C and LDL-C (P 〈 0.05). Three regression equations displayed that the level of serum lipid was positively related with TG and LDL-C, but negatively related with HDL-C; Protein made great impacts on LDL-C. Moisture was positively related with three kinds of serum lipid, and influenced LDL-C on the maximum content. ③ There was a correlation between body composition and creatinine (P 〈 0.05). CONCLUSION: Body composition is predictably related with serum lipid levels (especially TG, LDL-C and HDL-C) and creatinine.
