慢性乙型重型肝炎黄疸证患者外周血树突状细胞功能的研究

摘　　要：目的比较慢性乙型重型肝炎阳黄和阴黄证患者的外周血树突状细胞功能。方法体外培养阳黄与阴黄患者外周血树突状细胞,流式细胞仪检测其表面分子CD80、CD86、CD40、HLA-DR、CD1a的表达,MLR中刺激T细胞的增殖和分泌IL-12、IFNα的能力。结果阴黄患者的DC表面分子CD86的表达较阳黄组下降明显(P<0.05)。其他表面分子,如CD40、HLA-DR、CD1a和CD80,阳黄与阴黄二者无明显差异;经TNFα刺激后,上述各表面分子的表达均有上调,但阴黄组CD80、CD86、HLA-DR上调的幅度低于阳黄组(P<0.05)。阴黄患者组自身T淋巴细胞增殖的能力较阳黄患者组为低,但差异无统计学意义(P>0.05);经DC刺激后,阴黄患者的T淋巴细胞的增殖效果明显低于阳黄组(P<0.05)。阳黄和阴黄患者的IL-12和IFNα水平未经TNTα刺激前无明显差异,经TNTα刺激后两组的IL-12和IFNα水平较同组未经TNTα刺激者明显升高(P<0.05)。但阴黄组上升的幅度低于阳黄组(P<0.05)。结论慢性乙型重型肝炎阳黄和阴黄患者均存在明显外周血DC功能障碍,运用TNFα激活可提高DC的部分功能,阴黄患者的外周血DC细胞功能低下较阳黄患者更为明显。Objective To study the function of dendritic cells（DCs） in the patients with both yang and yin jaundice of chronic severe hepatitis B in TCM. Method The ceils in peripheral blood monocytes（PBMCs） were isolated and cultured in vitro from the patients. The expression of CD80, CD86, CD40, HLA-DR and CDla were determined with flowcytometry, and the level of IL-12 and IFNα produced by DCs were analyzed by ELISA. Result The expression rates of CD86 in Yin jaundice group was 48.4±10.1% and significantly lower than that in Yang jaundice group; the difference between any other two groups on the other phenotype（CD40/ CD80/CD1a/HLA-DR） was not significant. After stimulated with TNFα, expression rate of the phenotype in DCs was higher, but the length of CD80, CD86, HLA-DR in Yin jaundice group were lower than those in the Yang jaundice group. The generation of autologous T cells in Yin jaundice group was less than that in Yang-jaundice group（P〈0.05）. There was no difference between the levels of IL-12 and IFNα before stimulated with TNFα in both Yang and Yin jaundice groups and both significantly difference after stimulated. Conclusion There exists the obvious dysfunction of DC in peripheral blood in patients with chronic severe hepatitis B, but the function can recovered partly stimulated with TNFα Yin jaundice is less than that with Yang jaundice.
