赖脯胰岛素治疗40例应激状态Ⅱ型糖尿病患者临床观察

摘　　要：目的评价赖脯胰岛素(Lispro)对应激状态Ⅱ型糖尿病患者的疗效与安全性。方法对2003年7月～2005年12月莞城医院的80例应激状态Ⅱ型糖尿病患者随机分两组,分别用赖脯胰岛素(Lispro)和人胰岛素(诺和灵R)治疗,并于治疗前后测定空腹血糖、餐后2小时血糖,记录赖脯胰岛素及人胰岛素的用量、低血糖发生率。结果治疗后两组患者空腹及餐后血糖均得到良好控制,但赖脯胰岛素组餐后血糖优于人胰岛素(诺和灵R)组,每天所需Lispro剂量与常规重组人胰岛素相同,低血糖的发生率较人胰岛素(诺和灵R)组明显减少。结论Lispro在改善应激状态下Ⅱ型糖尿病患者的血糖控制的同时低血糖发生率低。Objective To assess the efficacy and safety of insulin Lispro in type Ⅱ diabetic patients in stress. Methods 80 type 2 diabetic patients in stress were divided into two groups in Guancheng Hospital from 2003-7 to 2005-12.One group was treated by insulin Lispro. Another was treated by regular human insulin（ Nor R）. The patients in two groups were checked fasting plasma glucose（FBG）,2h postprandial glucose（after a standardized meal） before and after treatment ,and marked the total daily insulin dose and the number of hypoglycemic episodes of the two groups. Results After treatment ,the levels of FPG and 2h postprandial grucose were decrease significantly in two groups, but the 2h postprandial glucose of the insulin Lispro group was better than the regular human insulin group. The dose of insulin hispro was not changed ,compared with the dose of regular human insulin at baseline .The incidence of hypoglycemia was decreased with Lispro. Conclusion Insulin Lispro is an effective agent for good glucose control with fewer hypoglycemic episodes in type Ⅱ diabetic patients in stress.
