摘  要:目的:探讨B族维生素(叶酸、B6和B12)治疗对降低脑卒中病人血浆总同型半胱氨酸(tHcy)的效果。方法:收集脑卒中患者120例为病例组,选择120例性别、年龄和种族等与病例组具有可比性的无脑卒中者作为对照组,采用以荧光偏振免疫检测技术为基础的自动化免疫检测方法测定血浆tHcy。然后脑卒中组病人被随机给予安慰剂或维生素。采集空腹血,并在1年内检测血浆tHcy水平。结果:脑卒中组血浆tHcy水平(17.82±4.21)μmol/L,显著高于对照组(13.82±3.59)μmol/L(P<0.05)。治疗前脑卒中患者两组中tHcy平均水平相似(安慰剂组17.54μmol/L;维生素组17.96μmol/L; P>0.05);治疗后1年内,安慰剂组tHcy平均水平是18.63μmol/L,维生素组12.38μmol/L(差值6.25μmol/L,P<0.001)。结论:血浆tHcy水平升高与脑卒中的发生密切相关,维生素治疗能降低脑卒中患者的平均tHcy水平。Objective:To study the efficacy of vitamin therapy (folic acid, vitamin B12, and B6) on lowering plasma total ho mocysteine(tHcy) in stroke patients. Methods:One hundred and twenty patients with stroke were included as the case group in this study,while 120 subjects without stroke matched with the case group for sex and age were collected as the control group. Plasma tHcy was measured by an automated immunoassay method based on fluorescence polarization immunoassay technology. Patients were randomized to receive either placebo or vitamins. Fasting blood samples collected at baseline and at 1 year were assayed for levels of plasma tHcy. Results: The plasma tHcy level of the case group was significantly higher than that of the control group(17.82 ±4. 21 )μmol/L vs. (13.82±3. 59)μmol/L,P〈0.05. Mean baseline tHcy was similar in the 2 groups of stroke patients (placebo 17.54 micromol/L; vitamins 17. 96 micromol/L; P〉0.05)at preteratment. At 1 year, mean tHcy level was 18.63 micromol/L in the placebo group compared with 12. 38 micromol/L in the vitamin group (difference 6. 25 micromol/L; P〈0. 001 )after teratment. Condusion:Increased plasma tHcy levels closely correlate with the occurrence of stroke, vitamin therapy can reduce mean tHcy levels in stroke patients.
