高血压左心室重构的超声心动图特征

摘　　要：目的:观察高血压患者心脏形态、结构,认识左心室重构、类型及特点。方法:对240例高血压患者和60例正常人进行超声心动图检查,按照Ganau的方法判断左心室重构及构型分型,进行统计分析。结果:高血压组中左室正常构型者22.50%,向心性重构者36.25%,向心性肥厚者32.50%,离心性肥厚者8.75%。离心性肥厚者的心指数和每搏指数高于对照组和向心性重构者;射血分数和左室短轴缩短率向心性重构者增高,离心性肥厚患者值低;高血压组E/A比值降低,尤其是向心性肥厚明显。结论:高血压患者发生左心室重构,有不同几何构型和特点。Objective： To observe the cardiac structure, characterization of remodeling of left ventricle and geometric patterns of the patients with essential hypertension. Methods： 240 cases of the patients with essential hypertension were examined by echocardiography compared with 60 healthy subjects according to methods used by other specialists. Results： There was remodeling of left ventricle in the hypertension, the patterns of remolding were normal, concentric remolding, concentric hypertrophy, eccentric hypertrophy, which accounted for 22.50%, 6.25%, 32.50% and 8.75% respectively. The cardiac index, stroke volume index, ejection fraction ,fraction of shortening, E/A ratio and so on were abnormal in the patients with hypertension, which correlated with different geometric patterns respectively. Conclusions： Remodeling of left ventricle occurs in hypertension, which has different geometric patterns and impairs heart functions.
