经胸超声心动图在先心病介入封堵术中的应用

摘　　要：目的:探讨经胸超声心动图在先心病房间隔缺损、室间隔缺损、动脉导管未闭介入封堵术术前、术中、术后的应用价值。方法:对房间隔缺损12例、室间隔缺损9例、动脉导管未闭10例(其中1例同时有房间隔缺损、室间隔缺损、动脉导管未闭;1例同时有房间隔缺损、室间隔缺损)经胸超声心动图检查条件符合而行Amplatzer封堵器封堵治疗。结果:28例全部封堵成功,其中1例动脉导管未闭术后1月内有残余分流,第3月复查残余分流消失;1例大室间隔缺损(34mm)术中有少量中心性残余分流,术后第3天复查残余分流消失。1例患者术后第2天出现交界性心动过速,1例患者术后第3天出现频发多源室性早搏,均经药物治疗后消失。结论:超声心动图在先心病介入封堵术中对术前病例的选择、术中指导监测、封堵器型号选择和术后随访疗效评价等都具有十分重要的价值。Objective：This paper is to explore the clinical value of the transthorae.ie echoeargraphy in the transcatheter closure for atrial septal defect（ ASD ）, ventricular septal defect （ VSD ）, and patent ductus arteiosus （ PDA ） before, during, after the operations. Methods： After color Doppler ecbocardiography, 12 cases of ASD, 9 cases of VSD,and 10 eases of PDA（among which one ease had all ASD,VSD,PDA and one case had ASD and VSD ） were treated with surgical treatment of Amplatzer occlusion operations. There were 11 male cases and 17 female cases, their ages ranging from 3 to 65 years with the average age of 20.21. Results： 28 cases were successfully treated with Amplatzer occlusion devices except for one PDA case with the residual shunt after the operation. One case with large ASD（34mm ） shows small amount of residual shunt in the center. After one month of the operation, the re-examination shows no sign of the residual shunt. One case shows interboundary tachycardia on the second day after the operation. Another case shows a frequent multi-source ventricular proiosystole three days after the operation, both cases are cured by medicine treatment.Conclusion：Transthoracic, Ecboeargraphy is valuable in the choice of eases for Amplatzer occlusion operations, in supervising operations, in choosing the occlusion devices, and in follow-up examinations.
